Plenty of online pharmacies sell monitored substances, such as opiates, without a patient prescription. Commonly called ‘No Prescription Websites’ (NPW), and oftentimes misinterpreted as being illicit or illegal and are found through various persistent online searching strategies (Unnikrishnan). Popular keywords for such searches include: “online pharmacies no prescription” or “get drug no script”. &lt;br /&gt;
&lt;br /&gt;
A 2008 study estimated the rising online drug trade at a record $12 billion, far surpassing any amount recorded before the turn of the century. The study also identified &quot;fraudulent online pharmacies and business-to-business exchange sties,&quot; claiming that China had 31 percent of the sites, the US 26 percent, and India 19 percent. Of those three countries, India has become a major producer of pharmaceutical opiates, with over 25,000 pharmacies in a population of over a billion people (Charles). The industry began its growth to world status 1980s and 1990s.  Opium and its derivatives, sanctioned for world health requirements and medical use, are typically manufactured through tight government control in India.  The Indian government manages organizational production involving thousands of small farm poppy fields. The poppy field are presently managed in the two states of Rajasthan and Madhya Pradesh. India actually exports over 70 percent of its raw opium mainly to the United States, Japan, France, and Sri Lanka.&lt;br /&gt;
&lt;br /&gt;
No matter how tight the government control may be, the Internet enhances the growth of both licit and illicit online pharmacies of India&#39;s opium, allowing the advantages of rapid global communication and ready distribution, while offering competitively low prices. Detection and prosecution of the illicit sites remain increasingly difficult as it is virtually impossible to decipher many sites’ real origins, since the Internet can be used to mask identity. That&#39;s why it&#39;s so important to find a reliable online pharmacy because any website has the immediate ability to switch domain name servers and change their web addresses. Also, credit card companies and other forms of online payment enable money exchanges using globally disguised identities and sources. &lt;br /&gt;
&lt;br /&gt;
Some of these online pharmacies, are legitimate and represent major global corporations, selling monitored substances to everyday consumers in Europe and the US. Some even require some form of account registration on the site, asking for email addresses.  Most of them provide an Indian address and email contact address, at least. They were mainly gelled from online directories, upon which they themselves usually form layers of ad-search linking sites. A user, searching for online pharmacies, will sometimes land upon a &quot;trade&quot; site, which specifies the various pharmaceuticals by flags of the country of origin and a sample listing of their medications. A search engine on the page locates those companies in India that supply codeine, morphine, buprenorphine, and others.&lt;br /&gt;
&lt;br /&gt;
I have an informative list of Indian pharmacies (both reliable and non-reliable), which includes the home site that will either show corporate headquarters in India or a temporary site asking for registration. A second ‘trade’ web is provided where the company has a summary report showing available an opiate derivative. We believe that it takes knowledge and understanding to make wise decisions about the prescription meds that we take from online pharmacies. The reality is that drug products sold via trustworthy online pharmacies are approved by the FDA whether they are brand name or generic. &quot;Most people believe that if something costs more, it has to be better quality. In the case of generic drugs, this is not true,&quot; says Gary Buehler, Director of FDA&#39;s Office of Generic Drugs. &quot;The standards for quality are the same for brand name and generic products.&quot;&lt;br /&gt;

Health professionals and consumers can be assured that FDA approved generic drugs have met the same rigid standards as the innovator drug. To gain FDA approval, a generic drug must:&lt;br /&gt;
&lt;ul&gt;&lt;li&gt;contain the same active ingredients as the innovator drug (inactive ingredients may vary)&lt;/li&gt;
&lt;li&gt;be identical in strength, dosage form, and route of administration&lt;/li&gt;
&lt;li&gt;have the same use indications&lt;/li&gt;
&lt;li&gt;be bioequivalent&lt;/li&gt;
&lt;li&gt;meet the same batch requirements for identity, strength, purity, and quality&lt;/li&gt;
&lt;li&gt;be manufactured under the same strict standards of FDA&#39;s good manufacturing practice regulations required for innovator products&lt;/li&gt;
&lt;/ul&gt;The Generic Drug is a drug which is bioequivalent to a brand name drug with respect to pharmacokinetic and pharmacodynamic properties. Generic medicines must contain the same active ingredient at the same strength as the &quot;innovator&quot; brand, be bioequivalent, and are required to meet the same pharmacopoeial requirements for the preparation. By extension, therefore, generics are identical in dose, strength, route of administration, safety, efficacy, and intended use.&lt;br /&gt;
&lt;br /&gt;
Thus, in some cases, the &quot;generic&quot; product is actually the brand product but inside a different box.&lt;br /&gt;
&lt;br /&gt;
The principal reason for the reduced cost of generic medicines is that these are manufactured by smaller pharmaceutical companies which do not invest in research and development into new drugs. The significant research and development costs incurred by the large pharmaceutical companies in bringing a new drug to the market is often cited as the reason for the high cost of new agents - they wish to recover these costs before the patent expires.Other reason for high prices is that every year pharmaceutical companies spend billions to market their products.&lt;br /&gt;
&lt;br /&gt;
When a pharmaceutical company first markets a drug, it is usually under a patent that only allows the reliable pharmaceutical company that developed the drug to sell it. This allows the company to recoup the cost of developing that particular drug. It will frequently cost millions of dollars to develop and test a new drug before it is approved for use. After the patent on a drug expires, any pharmaceutical company can manufacture and sell that drug. Since the drug has already been tested and approved, the cost of simply manufacturing the drug will be a fraction of the original cost of testing and developing that particular drug.&lt;br /&gt;
&lt;br /&gt;
So despite the strict standards imposed by the FDA for approval of generic drugs, and their enforcement of these standards, a number of misconceptions about generic drugs persist that we must disprove:&lt;br /&gt;
&lt;br /&gt;
MYTH: Generics take longer to act in the body.&lt;br /&gt;
FACT: The firm seeking to sell a generic drug must show that its drug delivers the same amount of active ingredient in the same timeframe as the original product.&lt;br /&gt;
&lt;br /&gt;
MYTH: Generics are not as potent as brand-name drugs.&lt;br /&gt;
FACT: FDA requires generics to have the same quality, strength, purity, and stability as brand-name drugs.&lt;br /&gt;
&lt;br /&gt;
MYTH: Generics are not as safe as brand-name drugs.&lt;br /&gt;
FACT: FDA requires that all drugs be safe and effective and that their benefits outweigh their risks. Since generics use the same active ingredients and are shown to work the same way in the body, they have the same risk-benefit profile as their brand-name counterparts.&lt;br /&gt;
&lt;br /&gt;
MYTH: Brand-name drugs are made in modern manufacturing facilities, and generics are often made in substandard facilities.&lt;br /&gt;
FACT: FDA won&#39;t permit drugs to be made in substandard facilities. FDA conducts about 3,500 inspections a year in all firms to ensure standards are met. Generic firms have facilities comparable to those of brand-name firms. In fact, brand-name firms account for an estimated 50 percent of generic drug production. They frequently make copies of their own or other brand-name drugs but sell them without the brand name.&lt;br /&gt;
&lt;br /&gt;
MYTH: Generic drugs are likely to cause more side effects.&lt;br /&gt;
FACT: There is no evidence of this. FDA monitors reports of adverse drug reactions and has found no difference in the rates between generic and brand-name drugs.&lt;br /&gt;
&lt;br /&gt;
Finally, the only &quot;unsafe and unhealthy&quot; thing about generics is the financial loss for the big pharmaceutical companies that want to protect their monopoly on our health, despite the necessity of some low income households for effective medical care.&lt;br /&gt;

Why would an article with such an esoteric title be of interest, of importance and relevancy to more than just Pain Management health care workers? Shouldn&#39;t such an article be of more importance and interest in a Professional Journal than to the educated public?  What is behind the idea of publishing it on the Internet (on a  blog about reliable online pharmacies no less) situated so that many more than just medical minds would come across it by happenstance?&lt;br /&gt;
&lt;br /&gt;
We must assume that this same population should, by now, understand how the median age of death, in our country as in others, has been prolonged.  We attribute this increasing life expectancy, over the previous few centuries, by all of the many scientific advances, by the formation of and stabilization of standardized-religion, and by the many laws of behavior, in-acted to prevent man&#39;s destruction of his/her fellow man/woman.&lt;br /&gt;
&lt;br /&gt;
For these and other reasons, the percentage of the population living over the age of 65 increases with every passing decade and century.  At this point, I hope that you can begin to better understand the importance of pain control in chronic non-cancer patients.  Since the percentage of the population over 65 is getting larger with each passing decade, it is becoming more common place to know or to know of an individual requiring pain control for a chronic non-cancerous problem.&lt;br /&gt;
&lt;br /&gt;
Breakthrough pain in cancer patients is associated with poor outcomes, a greater incidence of hospitalization, more difficult to treat pain syndromes, and, of course, the inevitable patient dissatisfaction with therapy.  None of the previous characteristics are found, in general, amongst the non-cancerous patients. &lt;br /&gt;
Breakthrough pain in non-cancerous patients is known to be prevalent, severe, and it shares several characteristics with cancer patients, such as that it is typically rapid in onset and frequently encountered.  Studies have shown that nearly three quarters of patients with non-cancer pain have significant episodes of pain.&lt;br /&gt;
&lt;br /&gt;
For the general population, is not important what the actual treatments are for pain control in chronic non-cancerous patients.  What is important for everyone to understand is that a growing part of people will be suffering with chronic non-cancerous pain.  We need to start to modify and/or drop, when appropriate, our misconceptions of individuals (young and old) that complain of chronic pain that proves to be non-cancerous in origin.  We must study how individuals on narcotic therapy preform when attempting to do normal daily functions.  Such functions would include work, play, and care-giving. I feel that we will be surprised how much of a normal life these individuals can live if given the chance.&lt;br /&gt;

When you&#39;re online and searching for products such as refrigerators or socks, the choices are endless, but at least you know when you are on a reliable Web site (i.e., K-Mart or Wal-Mart, etc.) versus one that seems a little suspect (i.e., Bob&#39;s Used Refrigerators or Nelson&#39;s Sock World). But when you are shopping for medication online and looking for a good, inexpensive deal, where do you turn? The trustworthy companies in the United States are all familiar and sometimes have discounted name brand pharmaceuticals. However, more often than not, due to the rising costs of healthcare in America, the prices for such medicine seems pricy compared to all the other foreign online pharmacies that sell generic drugs (sometimes for HALF the cost).&lt;br /&gt;
&lt;br /&gt;
But when you go to those websites most seem characteristics of scammers trying to get your social security number or credit card information. Most of the sites do not provide real customer service attributes, such as an online agent or 1-800 number. Rather they create an illusion that they are there to help you in your time of need with an obvious photo of an old smiling doctor, stethoscope perfectly placed around the neck. This rhetorical ploy sometimes leaves anyone searching for medications via an online pharmacy with a discomfort greater than whatever is ailing them in the first place. &lt;br /&gt;
&lt;br /&gt;
Like buying a car from a slimy dealer or a watch from a traveling circus, the age-old Latin caveat of buyer beware becomes prevalent when buying your pills online. That&#39;s why I created my blog http://www.onlinepharmacyfivethings.blogspot.com, which details five critical points for buying drugs online. I have scoured the world wide web, seeking out the best and most reliable online pharmacies, while reporting the nefarious ones to the BBB. I test the waters and make small purchases on some, while retesting those that I have had success with in the past, just to ensure they haven&#39;t faltered. The scary thing is that, on a weekly basis, I find the number of new online pharmacies cropping amidst an atmosphere where there is little-to-no repercussions for those that do not provide good customer service for online medications. &lt;br /&gt;
&lt;br /&gt;
But those that do provide decent customer care, get listed on my blog. And those that provide good care, but do not advertise on a regular basis, get my recommendation as well, whenever possible. Speaking of those online pharmacies that do not really advertise, one of the major criteria for good customer service is an online consultation that remains private and discreet. The online consultations are mainly through an easy-to-use online form that you fill out. After that, a certified physician will assess your condition quickly and inline with typical doctor-patient relationship guidelines. After this process, an order confirmation of some type is obtained. Then, a package arrives at your doorstep (although you have to be there to sign for it) only a few days later (depending on what kind of mail service you choose). Is it so hard to provide those kind of service for a reasonable price? Those that don&#39;t make the cut don&#39;t follow these above-stated criteria and should be blacklisted. Not only do they miss fulfilling the criteria, they sometimes actually steal from the credit card holder. One time I had an extra $200 charged to internet sites which are too erotic to mention here. Beyond that there are instances where your information becomes ripe for spammers, taking months to get your junk mail filter augmented in order to decrease the number of nasty emails.&lt;br /&gt;
&lt;br /&gt;
As it stands now, the scammers and the non-scammers get there day online. They fill a market-gap for many Americans without insurance (or for those with it, but still refuse to pay outrageous prices for name-brand pills). Maybe that&#39;s what we should really start investigating instead of those scammer online pharmacies. Maybe we should ask why does such a market exist in the first place? Is it because our own country&#39;s health-care system is imperfect? Soon thereafter, I was without insurance for the first time in my life. I thought that I could do without it for a while, until I finished community college and could land myself a paralegal job. But, as it turned out, I started to get very bad migraines, and Ibuprophen wasn’t cutting it. That’s when a friend told me about a drug called Imitrex. I did some research and found that it seemed right for me, so decided that I was going to give it a try. The only problem was that I didn’t have a doctor&#39;s prescription for it; in fact, I didn’t even have insurance to afford a doctor visit (imagine how expensive it would have been for the x-rays, etc.)&lt;br /&gt;
&lt;br /&gt;
That’s when I, like many others before me for various reasons, turned to the Internet for answers. I, of course, found a slew of online pharmacies that had what I was looking for, but most of them were over-priced. Adjusting my search query, I added inexpensive to my criteria of online pharmacies for which I was surfing. A large number of these came up as well, and upon first inspection, I immediately felt uneasy. Most were horribly designed and had membership fees. Also, being able to contact them seemed a challenge. I searched and searched until I found one that I felt semi-comfortable with. As things turned out, my drug did not get delivered to me from the online pharmacy that I choose and I ended up getting bombarded with emails. It did not take long before I filed a compliant with the BBB (Better Business Bureau). &lt;br /&gt;
&lt;br /&gt;
Still, that filing did not satisfy me. So, in conjunction with my studies and work (by this time I had finished school and gotten a job in Arizona at a Consumer Legal Aid Society), I decided to do some heavy-handed research on Internet pharmacies, particularly foreign internet pharmacies. What I found was a mass conglomerate of organizations that ranged from fly-by-night style businesses that steal from people to legitimate organizations that just didn’t have the technology, know-how, or money to create a pleasing Web site. Thus, in this blog, I decided to write what I learned about my research regarding how to at least find what you are looking for from a trustworthy site. The best I can hope for is that it helps you make a better, more informed decision.&lt;br /&gt;
