Dorel Juvenile Group Recalls Eddie Bauer Rocking Wood Bassinets Due to Infant Suffocation Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Eddie Bauer Rocking Wood Bassinets

Units: About 97,000

Manufacturer: Dorel Juvenile Group, of Columbus, Ind.

Hazard: The bottom locking mechanism can fail to lock properly if a spring is not installed, allowing the bassinet to tip to one side and cause infants to roll to the side of the bassinet. This poses a suffocation hazard to infants.

Incidents/Injuries: Dorel has received 17 reports of incidents with the recalled bassinets involving infants primarily younger than three months old. In two of the incidents, infants were reported to have had breathing difficulties after they rolled into the side of their bassinets.

Description: This recall involves Eddie Bauer Rocking Wood Bassinets. The bassinets have a dark brown wooden headboard and footboard and a cotton bassinet that comes in a variety of colors and patterns. "Eddie Bauer" is printed on a metal plate on the bassinet's footboard. These bassinets have a dial at the base of the footboard that locks and unlocks the rocking motion of the bassinet. Recalled bassinets have the following model numbers: 10632, 10639, 10832, 10835, 10839 and BT021. The model number is located under the mattress, on the top surface of the mattress support board or on the wash and care label.

Sold at: Target, Toys R Us and Sears stores nationwide and online at Ebay and other websites from December 2007 through January 2011 for about $150.

Manufactured in: China

Remedy: Consumers should immediately stop using the recalled bassinets and contact Dorel to obtain a free repair kit, which includes a spring and new instructions for assembly.

Name of Product: ValcoBaby "Joey" booster toddler seats for strollers

Units: About 975

Importer: Unique Baby Products USA LLC, d/b/a ValcoBaby, of Brooklyn, N.Y.

Hazard: The spring button mechanism securing the booster toddler seat to the baby stroller can disengage, allowing for the carried toddler to fall.

Incidents: The firm has received two reports of a child falling from the booster toddler seat after it disengaged from the stroller. No injuries were reported.

Description: The product is a booster toddler seat, both single and twin, designed to attach to Valco "Tri-Mode" and "Zee" strollers. Only booster seats with batch numbers 3111, 7819, 7822 and 7831 and model number TOD1058, TOD9109 and ZEE0649 are included in the recall. The batch and model numbers are printed on a label attached to the product frame. The seats have a black color fabric seat on white metal base frame.

Sold: Various juvenile product stores, websites and the firm's website valcobaby.com nationwide from June 2011 through June 2012 for between $80 and $100.

Description: The Happy Swing II is a fabric infant swing that comes in red and green with a tray and grab bar attachment. The model/style number included in the recall is "432" which is printed on a label on the frame of the swing. The fabric swing sits on a triangular frame and is battery operated.

Sold at: Juvenile products stores and CSN stores nationwide and online at Wayfair and Amazon.com from October 2010 through September 2012 for between about $80 and $130.

Hazard: The front openings between the tray and seat bottom and on the side openings of the high chair between the armrest and seat bottom can allow a child's body to pass through and become entrapped at the neck. This poses a strangulation hazard to young children if the belt is not engaged. In addition, exposed springs between the seat and armrest on both sides of the high chair can create a pinch hazard to the child.

Incidents/Injuries: None reported.

Description: This recall involves all Bistro high chairs with model/style number "255" which is printed on a tag attached to the back of the seat. The high chair was sold in blue or pink. The fabric on the seat has a polka-dot design and "Dream On Me" is printed on a label attached to the front of the white tray. The recalled high chair was manufactured in July 2011 and has a date code printed on a label on the back of the chair.

Sold at: Americas Kids, Kid Pro USA and independent juvenile specialty stores and online at Toysrus.com between November 2011 and September 2012 for about $75.

Name of Product: Graco Classic Wood Highchairs

Units: About 86,000 in the United States and 3,400 in Canada

Importer: Graco Children's Products Inc., of Atlanta, Ga.

Hazard: The high chair's seat can loosen or detach from the base, posing a fall hazard to the child.

Incidents/Injuries: Graco has received 58 reports of the high chairs' seats loosening or detaching from the base. There have been nine reports of children falling as the seat detached from the base, resulting in reports of bumps, bruises and scratches. Graco has received one report of a concussion in Canada.

Description: This recall involves all Graco brand Classic Wood Highchairs sold in three wood finishes. The high chair has a top seat, bottom leg assembly and removable tray. The high chair is sold with a beige fabric seat cover. Model number 3C00BPN, 3C00BPN TC, 3C00CHY, 3C00CHY TC, 3C00CPO or 3C00CPO TC is printed on a label on the underside of the seat assembly.

Sold at: Babies R Us, Burlington Coat Factory and other retail stores nationwide and at Target.com and Walmart.com and other online retailers between September 2007 and December 2010 for about $130.

Hazard: The night lights can overheat and smoke, posing a burn hazard to consumers.

Incidents/Injuries: Energizer has received nine reports of the night lights overheating, including three reports of minor property damage. No injuries have been reported.

Description: This recall involves Energizer LED rotating night lights. The night lights are white and have an LED light inside an adjustable dome on top. The night lights measure about 2 inches wide by 2 ½ inches long. Model number "ENLPLROT" and date codes between 0110 (for January 2010) through 0111 (for January 2011) are included in this recall. The model number is stamped onto the side of the night light. The date code is stamped in a circle on the back of the night light. The arrow points to the month of production (i.e. 12 = December) and the number positioned on either side of the arrow represents the year (i.e. "10" = 2010).

Sold exclusively at: Target stores nationwide from February 2010 through July 2012 for about $6.

Name of Product: Children's Water Bottle

Units: About 2,900

Hazard: The water bottle's spout can break off, posing a choking hazard to children.

Incidents/Injuries: H&M has received one report of an incident in England of the water bottle spout breaking off in a child's mouth as the child was drinking from the bottle. No injuries have been reported.

Description: The 16 oz. water bottles are pink plastic with a crackle design or blue plastic. The bottles have flip-top lids in coordinated pink and blue colors. "H&M Sweden" and "www.hm.com" are embossed on the bottom of the bottle. The recalled products were manufactured in June 2012. The water bottle contains the manufacture date in an embossed date clock on the bottom of the bottle. The inner circle on the clock contains the number 12 with an arrow between the two numbers. The arrow points at the number 6 in an outer circle of numbers.

Sold exclusively at: H&M stores with children's departments nationwide from July 2012 through August 2012 for about $5.

Manufactured in: Italy

Assembly instructions sold with the swings have item number JY1107 and SKU number 210020400 printed on the sheet.

Distributor: Specialized Bicycle Components Inc., of Morgan Hill, Calif.

Manufacturer: Kinesis, of China

Hazard: The front fork can break, posing fall and injury hazards to riders.

Incidents/Injuries: Specialized has received four reports of front forks breaking, resulting in facial fractures, head and shoulder injuries and cuts.

Description: This recall involves some 2008 and 2009 models women's and men's Globe model bicycles. Recalled models include the Globe Elite, Globe Sport, Globe Sport Disc, Globe Centrum Comp, Globe Centrum Elite, Globe City 6, Globe Vienna 3, Globe Vienna 3 Disc, Globe Vienna 4, Globe Vienna Deluxe 3, Globe Vienna Deluxe 4, Globe Vienna Deluxe 5 and Globe Vienna Deluxe 6 bicycles. The bicycles were sold in various colors, including gun, silver, black, khaki, navy, bone, blue, charcoal, burgundy and gold. The brand name "Specialized" is on the lower frame tube and the model name is on various locations on the bicycle frame.

Sold at: Authorized Specialized retailers nationwide from July 2007 to July 2012 for between $550 and $1,100.

Mr. Coffee Single Cup Brewers Recalled by JCS Due to Burn Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recallof the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell arecalled consumer product.

Name of Product: Mr. Coffee(r) Single Cup Brewing System

Units: 520,000 in the United States and 80,700 in Canada

Importer: Sunbeam Products Inc. d/b/a Jarden Consumer Solutions ("JCS"), of Boca Raton, Fla.

Hazard: A build-up of steam in the water reservoir can force the brewing chamber open and expel hot coffee grounds and water, posing a burn hazard.

Incidents/Injuries: JCS has received 164 reports of the brewing chamber opening due to steam pressure, including approximately 59 reports in the U.S. and two in Canada of burn injuries to consumers' face, upper torso and hands.

Description: The recalled coffeemaker comes in black with silver, red or white trim. It stands about 11 inches tall and has a Brew Now /Off button and a removable drip tray. The water tank is located on top of the unit towards the back. The model number is printed on the bottom of the brewer. Recalled model numbers are:

Death of Child Prompts Recall of Window Blinds by Blind Xpress
Two-year-old strangles in cord

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC), in cooperation with Blind Xpress of Livonia, Mich. is announcing the recall of about 139,000 custom-made vertical and 315,000 horizontal blinds. In 2009, a 2-year-old girl from Commerce Township, Mich. reportedly strangled in the loop of a vertical blind cord that was not attached to the wall or floor.

Blind Xpress custom vertical blinds have an adjustment cord that forms a loop that is not attached to the wall or floor. In some instances, this loop has a weighted device at the bottom. The custom horizontal blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. A child can become entangled in a cord loop and strangle.

This recall involves all Blind Xpress custom-made vertical blinds that do not have a cord-tensioning device that attaches to the wall or floor, as well as all horizontal blinds that do not have inner cord stop devices.

The blinds were sold at various blind specialty stores in Michigan, Ohio and Indiana from January 1995 through December 2011 for between $16 and $380. These blinds were manufactured in the United States.

Haier America Trading Agrees to $850,000 Civil Penalty for Failure to Report Defective Blenders

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) announced today that Haier America Trading LLC, of New York, N.Y., has agreed to pay a civil penalty of $850,000. The settlement agreement has been provisionally accepted by the Commission (4-0).

The settlement resolves CPSC staff allegations that Haier America failed to report immediately to CPSC, as required by federal law, a defect involving its blenders that resulted in nearly 60 incidents and an injury to a consumer's hand. The nut on the blender that holds the blade assembly can dislodge during use, allowing the blade assembly pieces to break apart, and/or crack the blender's glass jar, posing a laceration hazard to consumers.

Haier America sold the blenders through retail stores between October 2006 and October 2009. The company became aware of the incidents and injury between January 2007 and September 2009, yet did not file a full report to the Commission until October 2009.

Federal law requires manufacturers, distributors, and retailers to report to CPSC immediately (within 24 hours) after obtaining information reasonably supporting the conclusion that a product contains a defect that could create a substantial product hazard, creates an unreasonable risk of serious injury or death, or fails to comply with any consumer product safety rule or any other rule, regulation, standard or ban enforced by CPSC.

CPSC and Haier America announced a recall of nearly 54,000 blenders in December 2009.

In agreeing to the settlement, Haier America denies CPSC staff allegations of the defect and that it violated the law.

Simplicity Cribs Recalled by Retailers; Mattress-Support Collapse Can Cause Suffocation and Strangulation One Infant Death Reported WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing the recall of all Simplicity full-size cribs with tubular metal mattress-support frames. This recall includes fixed-side and drop-side cribs. These cribs pose a risk of serious injury or death due to entrapment, strangulation, suffocation and fall hazards to infants and toddlers. The crib's tubular metal mattress-support frame can bend or detach and cause part of the mattress to collapse, creating a space into which an infant or toddler can roll and become wedged, entrapped or fall out of the crib. CPSC has received a report of a one-year-old child from North Attleboro, Mass. who suffocated when he became entrapped between the crib mattress and the crib frame in April 2008. CPSC is aware of 13 additional incidents involving the recalled cribs collapsing due to the metal mattress- support frame bending or detaching, including one child entrapment that did not result in injury, and one child who suffered minor cuts to his head when his mattress collapsed and he fell out of the crib. CPSC staff urges parents and caregivers to stop using these cribs immediately and find an alternative, safe sleeping environment for their baby. Do not attempt to fix these cribs. Due to the fact that Simplicity and its successor, SFCA Inc., are no longer in business, CPSC has limited information about the number of cribs sold. All Simplicity drop-side cribs have previously been recalled for a hazard involving the drop side. Simplicity drop-side cribs could still be in use by parents or caregivers who are unaware of the recalls or by those who received a repair kit to immobilize the drop side from Simplicity when the firm was still in business. This recall involves all Simplicity cribs with tubular metal mattress-support frames, which include but are not limited to the following models: Crib Name / Model Number Aspen 4-in-1 / 8755 Chelsea Deluxe 4-in-1 Convertible Sleep System / 8324 Graco 4-in-1 Ultra Sleep System / 4600 Graco Aspen 3-in-1 / 8740 Simplicity Crib and Changer Combo / 8994 Simplicity Ellis Deluxe 4-in-1 Convertible Sleep System / 8676 Simplicity Nursery-in-a-Box Convertible Crib / 8910 Some model numbers are followed by letters, indicating the color or finish of the crib. The name "Simplicity Inc." or "Simplicity for Children" appears on a label on the crib's mattress-support frame and/or the crib's end panels. The cribs were manufactured in China. The recalled cribs were sold at Walmart, Target, Babies R Us and other stores nationwide for between $150 and $300. Consumers should contact the store where the crib was purchased to receive a refund, replacement crib or store credit. Important Message from CPSC: CPSC would like to remind parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Name of Product: Graco(r)-branded drop side cribs made by LaJobi

Units: About 217,000

Manufacturer: LaJobi Inc., of Cranbury, N.J.

Hazard: The drop side hardware can break or fail, allowing the drop

side to detach from the crib. When the drop side detaches, a

hazardous gap is created between the drop side and the crib mattress

in which infants and toddlers can become wedged or entrapped, posing

a risk of suffocation and strangulation. In addition, children can

fall from the cribs when the drop side detaches or fails to lock.

Incidents/Injuries: CPSC and LaJobi have received a total of 99

reports of drop side incidents, including hardware breakage and drop

side detachment. There were two incidents in which children became

entrapped in the gap created by the detached drop side. Both children

were freed by their caregivers. There were six reports of children

falling due to drop side failure, including one report of a mild

concussion.

Description: This recall involves LaJobi-manufactured Graco(r) wood

cribs. The full size cribs were sold in cherry, espresso, natural and

white finishes. The production date, item number, purchase order

number and finish name is printed on a label affixed to the footboard

or headboard. "LaJobi" and the crib model name are printed on a

product sticker located on the stabilizer bar or bottom rail of the

crib. Affected models are listed below.

Name of Product: Adventure Playsets Wooden Play Sets

Units: About 275,000 in the United States and 6,800 in Canada

Distributor: Adventure Playsets, of Amarillo, Texas

Hazard: The plastic coated lumber on the horizontal ladder (monkey bar/swing beam) can weaken over time due to rotting of the whitewood (spruce, pine and fir species), resulting in a fall hazard.

Incidents/Injuries: Adventure Playsets has received more than 1,400 reports of rotting ladders involving 16 injuries that resulted in nine emergency room visits. The injuries include two arm fractures, lacerations, scrapes, and bruises to children younger than 8 years old. Most of the reported injuries occurred when the swing came out of the monkey bar/ swing beam that had rotted.

Description: This recall involves wooden play sets with swings, slides and ladders. Each set has an overhead monkey bar ladder that acts as both the monkey bar and swing beam, and an end ladder coated with cranberry or green plastic. The instruction manual has the name "Adventure Playsets" and one of the following model numbers printed on the cover.

Wells’ Dairy Expands Voluntary Recall of Select Blue Bunny® Products

FOR IMMEDIATE RELEASE -- Le Mars, Iowa (January 30, 2009) – Wells’ Dairy, Inc., today expands its voluntary recall of its Blue Bunny Personals® Bunny Tracks® 8 fl oz. sold at retail and foodservice levels to include Lot Number 80030 “Best Used By” date 11/17/2009. The lot number can be found printed on the side of the carton. The affected product was shipped to the following states: Virginia, Iowa, Ohio, Nebraska, Washington, Tennessee, Michigan, Wisconsin, Indiana, North Carolina, Pennsylvania, and Oregon. This product contains peanuts from the Peanut Corporation of America (PCA), which is the focus of an ongoing Salmonella investigation.

Yesterday as a precaution Wells’ Dairy voluntarily recalled one lot of Blue Bunny Personals Bunny Tracks (80024 D19114 “Best Used By” date 09/11/09) and all Blue Bunny No Sugar Added Reduced Fat Bunny Tracks ice cream sold at retail and foodservice levels in 1.75 quart plastic containers with all codes displaying “Best Used By” dates prior to January 29, 2010. Best used by date is located on the bottom of the carton.

Consumers who have purchased the recalled products should return them to the store where they were purchased for a full refund. Anyone requiring more information should contact Wells’ Dairy consumer affairs at 1-800-331-0830.
