Simplicity Cribs Recalled by Retailers; Mattress-Support Collapse Can Cause Suffocation and Strangulation One Infant Death Reported WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission (CPSC) is announcing the recall of all Simplicity full-size cribs with tubular metal mattress-support frames. This recall includes fixed-side and drop-side cribs. These cribs pose a risk of serious injury or death due to entrapment, strangulation, suffocation and fall hazards to infants and toddlers. The crib's tubular metal mattress-support frame can bend or detach and cause part of the mattress to collapse, creating a space into which an infant or toddler can roll and become wedged, entrapped or fall out of the crib. CPSC has received a report of a one-year-old child from North Attleboro, Mass. who suffocated when he became entrapped between the crib mattress and the crib frame in April 2008. CPSC is aware of 13 additional incidents involving the recalled cribs collapsing due to the metal mattress- support frame bending or detaching, including one child entrapment that did not result in injury, and one child who suffered minor cuts to his head when his mattress collapsed and he fell out of the crib. CPSC staff urges parents and caregivers to stop using these cribs immediately and find an alternative, safe sleeping environment for their baby. Do not attempt to fix these cribs. Due to the fact that Simplicity and its successor, SFCA Inc., are no longer in business, CPSC has limited information about the number of cribs sold. All Simplicity drop-side cribs have previously been recalled for a hazard involving the drop side. Simplicity drop-side cribs could still be in use by parents or caregivers who are unaware of the recalls or by those who received a repair kit to immobilize the drop side from Simplicity when the firm was still in business. This recall involves all Simplicity cribs with tubular metal mattress-support frames, which include but are not limited to the following models: Crib Name / Model Number Aspen 4-in-1 / 8755 Chelsea Deluxe 4-in-1 Convertible Sleep System / 8324 Graco 4-in-1 Ultra Sleep System / 4600 Graco Aspen 3-in-1 / 8740 Simplicity Crib and Changer Combo / 8994 Simplicity Ellis Deluxe 4-in-1 Convertible Sleep System / 8676 Simplicity Nursery-in-a-Box Convertible Crib / 8910 Some model numbers are followed by letters, indicating the color or finish of the crib. The name "Simplicity Inc." or "Simplicity for Children" appears on a label on the crib's mattress-support frame and/or the crib's end panels. The cribs were manufactured in China. The recalled cribs were sold at Walmart, Target, Babies R Us and other stores nationwide for between $150 and $300. Consumers should contact the store where the crib was purchased to receive a refund, replacement crib or store credit. Important Message from CPSC: CPSC would like to remind parents not to use any crib with missing, broken or loose parts. Make sure to tighten hardware from time to time to keep the crib sturdy. When using a drop-side crib, parents should check to make sure the drop side or any other moving part operates smoothly. Always check all sides and corners of the crib for disengagement. Any disengagement can create a gap, which could fatally entrap a child. In addition, do not try to repair any side of the crib with tape, wire, rope or by other means. Infants and toddlers have died in cribs with makeshift repairs. Graco(r)-Branded Drop Side Cribs Made by LaJobi Recalled Due to

Entrapment and Suffocation Hazards

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in

cooperation with the firm named below, today announced a voluntary

recall of the following consumer product. Consumers should stop using

recalled products immediately unless otherwise instructed.

Name of Product: Graco(r)-branded drop side cribs made by LaJobi

Units: About 217,000

Manufacturer: LaJobi Inc., of Cranbury, N.J.

Hazard: The drop side hardware can break or fail, allowing the drop

side to detach from the crib. When the drop side detaches, a

hazardous gap is created between the drop side and the crib mattress

in which infants and toddlers can become wedged or entrapped, posing

a risk of suffocation and strangulation. In addition, children can

fall from the cribs when the drop side detaches or fails to lock.

Incidents/Injuries: CPSC and LaJobi have received a total of 99

reports of drop side incidents, including hardware breakage and drop

side detachment. There were two incidents in which children became

entrapped in the gap created by the detached drop side. Both children

were freed by their caregivers. There were six reports of children

falling due to drop side failure, including one report of a mild

concussion.

Description: This recall involves LaJobi-manufactured Graco(r) wood

cribs. The full size cribs were sold in cherry, espresso, natural and

white finishes. The production date, item number, purchase order

number and finish name is printed on a label affixed to the footboard

or headboard. "LaJobi" and the crib model name are printed on a

product sticker located on the stabilizer bar or bottom rail of the

crib. Affected models are listed below.

Hazard: The walkers can fit through a standard doorway and are not

designed to stop at the edge of a step. Babies using these walkers

can be seriously injured or killed.

Incidents/Injuries: No injuries or incidents have been reported.

Description: The recalled walkers are intended for babies 6 months

and older. The item number L-0809EN is printed on the side of the

box. The walkers are blue or pink with a white tray.

Sold at: Small independent retailers and discount stores in

California and Arizona from June 2009 through August 2009 for between

$25 and $28.

Manufactured in: Taiwan

Remedy: Consumers should stop using these walkers immediately and

return them to the store where purchased for a full refund.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following products. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Adventure Playsets Wooden Play Sets

Units: About 275,000 in the United States and 6,800 in Canada

Distributor: Adventure Playsets, of Amarillo, Texas

Hazard: The plastic coated lumber on the horizontal ladder (monkey bar/swing beam) can weaken over time due to rotting of the whitewood (spruce, pine and fir species), resulting in a fall hazard.

Incidents/Injuries: Adventure Playsets has received more than 1,400 reports of rotting ladders involving 16 injuries that resulted in nine emergency room visits. The injuries include two arm fractures, lacerations, scrapes, and bruises to children younger than 8 years old. Most of the reported injuries occurred when the swing came out of the monkey bar/ swing beam that had rotted.

Description: This recall involves wooden play sets with swings, slides and ladders. Each set has an overhead monkey bar ladder that acts as both the monkey bar and swing beam, and an end ladder coated with cranberry or green plastic. The instruction manual has the name "Adventure Playsets" and one of the following model numbers printed on the cover.

Description: This recall involves the Fiesta Masquerade and the Home Olympic 20- piece flatware sets. The Fiesta Masquerade flatware set is silver-colored and the handles have plastic decorative inserts in red, orange, yellow, green and three different shades of blue. The Home Olympic flatware sets are charcoal and the handles have decorative black inserts.

Hazard: When used as an activity table, the cap on one end of the product can loosen and fall off, posing a fall hazard to a young child.

Incidents/Injuries: Evenflo has received 11 reports of dislodged end caps which resulted in nine minor injuries, including bumps and bruises. Evenflo has also received a report of a dislodged end cap that resulted in a child suffering a broken collarbone in Canada.

Description: The recall involves Evenflo ExerSaucer Triple Fun stationary activity centers made between October 2006 and December 2008. The model number is 6231711. Stage 3 position involves converting the product into an activity table. No other ExerSaucer models use end caps and, therefore, no other models are included in this recall.

Sold at: Juvenile product and mass merchandise stores nationwide, including Toys 'R Us and Burlington Baby Depot, from October 2006 through February 2009 for about $120.

FOR IMMEDIATE RELEASE -- Le Mars, Iowa (January 30, 2009) – Wells’ Dairy, Inc., today expands its voluntary recall of its Blue Bunny Personals® Bunny Tracks® 8 fl oz. sold at retail and foodservice levels to include Lot Number 80030 “Best Used By” date 11/17/2009. The lot number can be found printed on the side of the carton. The affected product was shipped to the following states: Virginia, Iowa, Ohio, Nebraska, Washington, Tennessee, Michigan, Wisconsin, Indiana, North Carolina, Pennsylvania, and Oregon. This product contains peanuts from the Peanut Corporation of America (PCA), which is the focus of an ongoing Salmonella investigation.

Yesterday as a precaution Wells’ Dairy voluntarily recalled one lot of Blue Bunny Personals Bunny Tracks (80024 D19114 “Best Used By” date 09/11/09) and all Blue Bunny No Sugar Added Reduced Fat Bunny Tracks ice cream sold at retail and foodservice levels in 1.75 quart plastic containers with all codes displaying “Best Used By” dates prior to January 29, 2010. Best used by date is located on the bottom of the carton.

Consumers who have purchased the recalled products should return them to the store where they were purchased for a full refund. Anyone requiring more information should contact Wells’ Dairy consumer affairs at 1-800-331-0830.

Hazard: The zipper tabs on these vests fail to meet the children's torques test standards, posing a choking hazard to children.

Description: This recall involves the "Ruff Hewn" reversible vests for boys sold in toddler (style # F821235) and 4 through 7 sizes (style # F841235). The vests are brown and reverse to a camouflage print. "Ruff Hewn" is printed on the tag found in the left pocket of the brown side and on the price tag. The style number is printed on the price tag.

The name Stabilicers Lite appears in raised letters on the underside of the harness.

(Jan 21, 2009) – Chef Jay's Food Products today announced a voluntary recall of some of its branded food products that contain peanut butter because the products have the potential to be contaminated with Salmonella. The recalled products contain peanut butter manufactured by Peanut Corporation of America (PCA). As you may be aware, Peanut Corporation of America (PCA) voluntarily has recalled certain lots of its peanut butter and peanut butter paste because these products could be contaminated with Salmonella. Chef Jay's Food Products has identified (5) five items that contain the specified lots of peanut butter recalled by PCA. The following products, in all sizes and packages, with the "Best By" dates ranging from 06/Sept/09 thru 16/Jan/10 are included in the recall:

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. FOR IMMEDIATE RELEASE -- BATTLE CREEK, Mich. -- January 16, 2009 -- Kellogg Company today announced a voluntary recall of certain Austin® and Keebler® branded Peanut Butter Sandwich Crackers and select snack-size packs of Famous Amos® Peanut Butter Cookies and Keebler® Soft Batch Homestyle Peanut Butter Cookies because the products have the potential to be contaminated with Salmonella. No other products are involved in this recall.

On January 14, Kellogg announced a precautionary hold on the sandwich crackers while FDA and other authorities investigated Peanut Corporation of America (PCA), one of Kellogg's peanut paste suppliers for these crackers. PCA has expanded their earlier recall to include peanut paste and peanut butter, prompting Kellogg to immediately announce this recall and to include the above- mentioned cookie and cracker products.

(January 16, 2009) -- Peanut Corporation of America (PCA), a peanut processing company and maker of peanut butter for bulk distribution to institutions, food service industries, and private label food companies, today announced an expanded recall of peanut butter produced in its Blakely, Georgia processing facility as well as the voluntary recall of peanut paste produced in the same plant because these products have the potential to be contaminated with Salmonella. The company on January 13, 2009 previously announced the recall of 21 lots of peanut butter produced on or after July 1, 2008.

Today's announcement and voluntary recall affect all peanut butter produced on or after August 8, 2008 and peanut paste produced on or after September 26, 2008 at the Georgia facility. The peanut butter being recalled is sold by PCA in bulk packaging in containers ranging in size from five to 1,700 pounds. The peanut paste is sold in sizes ranging from 35 pound containers to tanker containers.

PCA is notifying customers who received the recalled product by telephone or in writing, as well as through the news media and a toll-free 24/7 hotline number. None of the peanut butter or peanut paste being recalled is sold directly by PCA to consumers through retail stores.

"Today, the FDA informed PCA that new product samples in unopened containers tested positive for Salmonella," said Stewart Parnell, President of Peanut Corporation of America.

The FDA has not yet confirmed the DNA fingerprints of these positive samples to match the strains causing the outbreaks of food borne illness in several states.

PCA is immediately stopping all production at the Blakely, Georgia facility and notifying its customers to recall and retain all affected product produced during these dates at this plant.

"We deeply regret that this product recall is expanding and our first priority is to protect the health of our customers. Our company has worked around the clock for the last week with federal regulators to help identify any potential problems. Our Blakely facility is currently not operating as we continue to work with federal food safety investigators,"Parnell said.

Eating food contaminated with Salmonella can result in abdominal cramping, diarrhea, and fever. Most people infected with Salmonella develop the symptoms 12 to 72 hours after infection. The illness usually lasts 4 to 7 days, and most people recover with treatment. However, in some persons, the diarrhea may be so severe that the patient needs to be hospitalized. FOR IMMEDIATE RELEASE -- WEST DES MOINES, IA -- January 17, 2009 -- Hy-Vee Inc. is voluntarily recalling the following products made in its bakery departments because they contain peanut butter that has the potential to be contaminated with salmonella: Peanut Butter Cookies, Monster Cookies, Peanut Butter Reese's Pieces Cookies, Peanut Butter Chocolate Chip Cookies, Lunchbox Reese's Pieces Cookies, Lunchbox Peanut Butter Cookies, People Chow Party Mix and Assorted Truffle Fudge. All sell-by dates are included in this recall. The products are sold in various packaging and quantities and have a Hy-Vee price label attached. All items should be destroyed or returned to Hy-Vee for a full refund.

The action was taken immediately after Peanut Corporation of America (PCA), the company that supplies bulk peanut butter to Hy-Vee, issued a recall of the peanut butter ingredient used to make the Hy-Vee bakery products. The recall is a precautionary step because of an unresolved nationwide outbreak of salmonella.

The identified items, sold in all Hy-Vee stores in Iowa, Illinois, Missouri, Kansas, Nebraska, South Dakota and Minnesota, have not been directly linked to the salmonella outbreak and there have been no reported cases of the illness.

Hy-Vee Bakery Manufacturing of Des Moines, Iowa, supplies stores with many of their baked goods. PCA recalled a number of lots of the product this week after health inspectors found salmonella contamination in an already-opened container of peanut butter at a nursing home in Minnesota. Hy-Vee had been shipped product from two of the lots. Production was suspended immediately and stores instructed to remove baked goods that contained peanut butter from store shelves.

In addition, Hy-Vee has sent samples of the peanut butter to an independent lab for testing. Company officials emphasize that all these measures are precautionary and they are keeping close watch on the situation and FDA's efforts to find the source of the outbreak. FDA and other regulatory agencies have indicated that Peanut Corporation of America (PCA) is the focus of an investigation concerning a recent Salmonella outbreak thought to be caused by tainted peanut butter. FOR IMMEDIATE RELEASE -- Madera, CA, January 22, 2009 – Due to the expanded recall of Peanut Butter Corporation’s peanut butter earlier this week, Rain Creek Baking Corporation announces a voluntary withdrawal of Sinbad and Rain Creek Baking Company branded dessert products produced with peanut butter because the products have a potential of being contaminated with Salmonella.

No illnesses have been reported in connection with the peanut butter desserts.

The recalled products contain peanut butter manufactured by Peanut Corporation of America (PCA). PCA is the focus of an investigation by the U.S. Food and Drug Administration (FDA) concerning a recent Salmonella outbreak thought to be caused by tainted peanut butter.

Items affected by recall are listed below. Items will be marked with either an “exp” (as in expiration date), “best before” date or a lot code. The expiration date or the best before date will read July 22, 2009 and prior. Lot numbers are 08182 sequentially through 08366 and 09001 sequentially through 09022. These lot numbers can be found on the bottom label next to the ingredient statement.

Items with an asterisk (*) are the only 2009 produced items (when looking for lot numbers). These products were sold in grocery, warehouse and other retail stores throughout the United States. It is possible that some products may have already been purchased by consumers and therefore anyone who has purchased the following products with the expiration dates listed should dispose of the product or return the product to the store for a full refund.

While none of the Sinbad and Rain Creek Baking Company branded products have been identified as contaminated or linked to any illness, Rain Creek Baking Corporation is requesting that the peanut butter dessert items be removed while the FDA continues its investigation to verify the source of a nationwide outbreak.

No other Sinbad® or Rain Creek Baking Corporation® products or flavors are included in this recall.

FOR IMMEDIATE RELEASE -- Garner, IA, January 22, 2009 – Aspen Hills, Inc. is announcing a voluntary recall of certain cookie dough products because the dough may have the potential to be contaminated with Salmonella. The recalled cookie dough contains peanut butter supplied by Peanut Corporation of America, which is the subject of the ongoing investigation concerning the recent Salmonella outbreaks. Neither Aspen Hills nor its customers have received any reports of illness connected to any of these products.

The company has already notified all of its customers who have received the products in question and directed them to remove the affected products from their distribution. The products are sold nationwide in 3 lb. pails, and 3 lb. corrugated boxes to distributors who are involved in fund raising.

FOR IMMEDIATE RELEASE -- MINNEAPOLIS, MN -- January 20, 2009 – Nash Finch, the operator of stores named: SunMart Foods, Econofoods (excluding Wisconsin stores in Sturgeon Bay, Clintonville, Marquette, Holton and Iron Mountain), Prairie Market, Avanza Supermarket, Food Bonanza, Wholesale Food Outlet, Family Fresh Market, Family Thrift Center, and Pick'n Save (Ohio stores in Van Wert and Ironton only) is taking the precautionary measure of voluntarily recalling the following products made in the bakery departments of its corporate operated stores because they contain peanut butter that was supplied by Peanut Corporation of America (PCA) and has the potential to be contaminated with salmonella: Peanut Butter Cookies, Peanut Butter Chocolate Chip Cookies, Mini Peanut Butter Cookies, Monster Cookies, Peanut Butter Grand Brownies, Peanut Butter Blossom Cookies, Puppy Chow snack mix, Peanut Butter Rolls, Peanut Butter Rice Crisp Bar, Special K-Bar, Scotcheroos, Rolls and Cakes Iced with Peanut Butter Cream. All sell-by dates are included in this recall. The products are sold in various packaging and quantities and have a price label attached bearing one of the above identified store names. All items should be destroyed or returned to the store where purchased for a full refund.

The action was taken after Peanut Corporation of America (PCA), the company that supplies bulk peanut butter to Nash Finch, issued a recall of the peanut butter ingredient used to make the Nash Finch bakery products. The recall is a precautionary step because of an unresolved nationwide outbreak of salmonella.

The identified items, sold in Nash Finch stores in Colorado, Ohio, Iowa, North Dakota, South Dakota, Wisconsin, Nebraska and Minnesota, have not been directly linked to the salmonella outbreak and there have been no reported cases of the illness from the identified items.

PCA has recalled a number of lots of the product after health inspectors found salmonella contamination in an already-opened container of peanut butter at a nursing home in Minnesota. Nash Finch had been shipped product from several of the lots. Production of these Nash Finch items has been suspended and stores instructed to remove baked goods that contained peanut butter from store shelves. No other Nash Finch bakery products are involved in this recall.
