This document covers all of the stages of the application process from the SHA-led Trust Development Phase and the Secretary of State Support Phase through to Monitor’s assessment process and the application outcome. Please note that since this document was published, updates have been made to Chapters 5 and 6 and Monitor’s appendices in the Guide for Applicants published in November 2008. This update supersedes the relevant chapters in the November 2008 Guide for Applicants.

This document provides an update to Chapters 5 and 6 and Monitor's appendices of the 2008 Guide for Applicants and supersedes the relevant chapters in that version. Appendix B12 of this document was updated in June 2011 to bring it in line with the Compliance Framework 2011/12.
