“How did you cut your arm?” a parent asked her daughter. “It happened during a gym class at school,” her teenage daughter replied. But many reports are finding that these marks are self-inflicted. Why would teenage girls, and even a royal princess, want to injure themselves?

It’s called the “Cutting Disease” and experts say it has been on the rise for the last 10 years. Self-mutilation is not a new disorder. Dr. Amando Favazz, a psychiatrist at the University of Missouri, the guru on self-mutilation, refers to a reference from the gospel of St Mark, chapter five, in which a man cries out and cuts himself with sharp stones.

Piercing one’s ears and tongue is one thing. But intentionally cutting yourself regularly with razor blades, burning the skin with cigarettes, pulling out hair or breaking bones is to most people an unthinkable way to act. Normally most of us try to prevent injury.

Self-cutting involves both the rich and poor. But nearly 100% of the time it’s females who are tormented by this disorder.

The Mayo Clinic reports “there’s no single or simple cause to explain this problem as it’s often triggered by a mix of emotional disorders and an inability to cope in a healthy way with psychological pain.” In short, a whole basket of emotional problems.

Experts stress that self-mutilation is vastly different from suicide, where the intention is to end one’s life. Rather, regularly cutting the skin is an attempt to use the body to express that their physical pain is nothing compared to the awful feelings they bear inside. And that self-injury is due to a mixture of anger, fear, anxiety, loneliness, guilt or emptiness that engulfs the lives of sufferers.

One psychiatrist remarked, “Initially, cutting helps to relieve their intense anxiety. But they quickly realize that the problem still exists and they feel ashamed, guilty and try to hide the scars.”

When analyzing the histories of such tormented people, several underlying problems stand out. The majority have endured an unhappy childhood for a variety of reasons. Some were maltreated by abusive parents. Others faced the emotional turmoil of seeing parents divorce and being shuttled back and forth between unhappy households. Or there’s a recurrent history of drug, alcohol or sexual abuse.

A prime example is the late Princess Diana, who witnessed the divorce of her parents, was shuttled between households and later bound by an unhappy marriage to Prince Charles. She then shocked the world in 1995 during a television interview with the BBC, revealing that she was a self-injurer who cut her arms and legs. She explained, “you have so much pain inside yourself that you try and hurt yourself on the outside because you want help”.

In “Diana: Her True Story”, a biography about her life, she admits she had often thrown herself against a glass cabinet at Kensington Palace, slashed her wrist with a razor blade and a serrated lemon slicer. Once during an argument with Prince Charles she cut her wrist and thighs with a pen knife. On another occasion, during a plane flight, she locked herself in the bathroom, cut her arms and smeared the blood on the cabin walls and seats.

When patients are asked why they self mutilate they provide a number of different reasons. One remarked poetically, “I know it’s wrong, but it feels so right, so I’ll just add another scar tonight.” Others say it gives them a sense of control over their body. Or as Tracy Thompson wrote in “The Beast: A Reckoning With Depression”, “It’s better to inflict injury on myself than let other people do it.”

There’s no magic bullet to ease the internal turmoil in these people as it’s never easy to treat depression, anxiety, obsessive compulsive disease and other emotional troubles. Besides, self-cutters hide their secret for years by wearing long sleeves or using other deceptions.

But I hope this column will alert parents who see suspicious cuts and bruises. They should see that their teenage daughter receives prompt treatment. The longer cutters cut, the more difficult the treatment.

I have been seeing the same therapist since the beginning of 2010 and everything was going well until June when my meds were beginning to slip and I felt like I was losing control. I knew a med switch was coming so we put off any appointments for a month and then we would decide where to go from there. Well that day is tomorrow and to be honest I have no idea what I am going to say.

My brain is still dealing with the new medication and chances are it will be increased in early September at the next mental health doc appointment. My mood swings can be pretty severe at times and the depression hits levels that I prayed would never return plus a sleep disturbance that is just adding to the entire dilemma. When my brain is not chemically fixed it becomes easy to slide into old behavior patters and the main one tends to be I shut down due mostly to paranoia even though I know it is not real but when my brain is racing it is difficult to control.

I am not thinking clearly so maybe the best solution would be to postpone therapy until I am more grounded and clear headed.

The last session I was in a bad place and the therapist was concerned that I would do something drastic so she kept asking the same questions to determine my safety level and I gave her the answers that she wanted to hear. There are certain parts of my illness that I do not share easily and one of them is my overall mental state. In the past when I am running through a rough spell I will fight it out and if I am unable to do so I tend to lose control and before I know it I am back on the psych ward. I am unable to put my health or sanity in someone elses hands which is not a surprise to anyone who knows me but it is an area that I have to fix.

Tomorrow meeting should be interesting and hopefully something positive comes out of it. The new drug has been in play for almost four weeks and it is almost certain it needs to be raised. No idea what the equivalency between Pristiq and Effexor is but 50mg of Pristiq has more side effects than positive features so if it is increased to 100mg I am hoping it will be similar to the effect I received with a 150mg of Effexor. I am rambling and it looks like my sleeping med that I took hours ago is starting to kick in. take care

ADHD: Who makes the diagnosis?

As a toddler, Ian Barrier got expelled from day care.

"They just said that he was all over the place, he couldn't handle the structure, they didn't have the staff or the skills to deal with it," said his mother, Amy Barr. "They said, 'We think he has ADD or ADHD' and I'm like, 'What is that?"

Ian, now 11, and his 9-year-old brother Aidan are just two examples of some 5 million children in the United States who have received the diagnosis of attention deficit hyperactivity disorder (ADHD), a condition marked by impulsive behavior and a lack of focus.

But although this is a medical condition with medical treatments available, often doctors aren't the ones suggesting a diagnosis. More on ADHD

Many parents begin their struggles with treating their children's ADHD the way that Barr did: with a suggested diagnosis from a school or day care setting. That's a problem, doctors say, when there could be many other factors contributing to a child's behavior.

For a teacher to suggest that a child has ADHD is "inappropriate and dangerous," says Dr. Elizabeth Roberts, child psychiatrist in Murrieta, California. Depression, anxiety and abuse are all possibilities in a child's life that could lead to attention problems, Roberts said. That means that many children are receiving medication for the wrong problem.

Roberts wants to say to all educators: "There are many, many diagnoses that cause these problems, including abuse and depression and anxiety. So please, withhold your judgment."

But Barr is happy that her son's day care center mentioned the condition to her. It explained Ian's behaviors that she had been excusing with the adage "boys will be boys." The center also directed her to a facility where he could get tested.

"I'm just glad that they brought it up, because I was living in it and didn't know any better," she said.

In Barr's case, having gotten her kids tested for ADHD even before school, the educational system has been supportive. But the question of medication has plagued her, with years of trying different combinations of drugs -- sometimes four or five at a time.

Despite medication, Ian pulled the fire alarm at his fourth day care center, dispatching the same fire department that would get him out of a tree at age 9, Barr said. And both brothers have spent time in a psychiatric ward at a children's hospital; for the younger one, it may have been because of overmedication, Barr said.

"You go through all of that, and it's just various different medications, and then you start to feel guilty: I'm drugging my kid," said Barr, of Fleming Island, Florida. Her boys are now both on just one medication -- Vyvanse (lisdexamfetamine) -- for focusing in school.

All too often, parents come to pediatrician Dr. Claudia Gold's office asking for a prescription for ADHD, based on a recommendation from school. When she consults with parents alone, however, she's likely to hear stories of trauma: a death in the family, an abusive relationship, and other life experiences that the teacher knew nothing about.

"I think that sometimes folks want an immediate answer and they want to help a child as quickly as possible," said Cheryl Rode, Director of Clinical Operations at the San Diego Center for Children in California. "Medication is quick and easy but it's not the answer alone for working with kids who have ADHD."

Experts agree that ADHD does exist as a real disorder, and that some children really do benefit from medication. Studies have shown a biological basis to the disorder and a genetic component, suggesting it can be passed down in families.

But it's not the teacher's place to make diagnoses, or to recommend medication, Gold said.

Teachers and related school personnel have an important role in identifying learning and social challenges faced by students, including those with ADHD, says Clarke Ross, CEO of CHADD (Children and Adults with Attention Deficit / Hyperactivity Disorder). But teachers should never give a diagnosis for the purpose of medication use, or advise the use of medications, he said.

Alana Morales, of Thornton, Colorado, is one of those teachers who has brought up the subject of ADHD testing with many parents. She doesn't tell parents to medicate their kids, but brings up the subject of getting children tested because she thinks it's important for parents to know.

"You have to be so careful because, again, we are not doctors," she said. "But does that mean we don't recognize it? No."

Not every child with ADHD needs medication, doctors say, and Morales said some students benefit more from counseling, special adjustments in school, a tutor, or a different environment for doing homework. It's helpful for parents to get informed about the condition and become advocates for their children -- but some may make the situation worse by having a closed mind to interventions and denying that there's anything wrong, she said.

"You're really setting them up for failure, because it's not a crime, it doesn't mean you're less of a parent if your child learns differently," she said.

Rode agrees with Roberts that teachers should not recommend medication, but thinks that teachers have an excellent frame of reference for what is typical or normal development.

Also the parent of two children with ADHD, Morales has specific strategies she uses when there are kids with ADHD in her class: don't seat them together, put them on the edges of the classroom so she can see them better, and sometimes modify assignments.

Providing an environment conducive to helping kids with conditions such as this is getting harder as budget-tightening schools cut back on teachers, Morales said.

Last year, teaching at a public school in Arizona, Morales had one classroom with 37 students, which included at least four kids with ADHD and one with Asperger's syndrome. And teachers whose job it is to provide extra help to those kids get responsibility for more students, leaving them with less time for each one.

Roberts estimates that only about 10 to 20 percent of children who receive the diagnosis of ADHD actually need medication.

While parents like Barr have tried to inform themselves as much as possible about ADHD and treatment options, some doctors say the condition gets misapplied because it is a quick and easy catch-all diagnosis for a variety of behavior problems, with drugs available to treat it.

Parents are also culprits, and bear some responsibility for their children's behavior, Roberts said. She thinks a lot of kids just need better structure and discipline in the home.

"Parents want to leap-frog over the parenting job and get right to the grandparenting," she said. "Everyone wants to have fun with their kids and everyone wants to be popular with their kids like Grandma is. But the reality is: Someone has to be the bad guy or the kid never learns."

Morales recommends that parents stay involved in their child's school lives by helping them with homework, assisting with organizing their papers, and writing notes to teachers so that everyone is on the same page.

On behalf of her own two children with ADHD, Morales is writing letters now to their teachers explaining how her kids learn differently and what accommodations have been made in the past (they are both on medication). She is also asking the teachers to let her know about any problems this school year.

Looking back, Barr wishes that when her children first got the ADHD diagnosis and then started school, she could have had a teleconference with the school and the psychiatrist at the same time. Instead, she had to relay what the teachers said about her children's behavior to the psychiatrist, and then the psychiatrist's treatment decisions back to the school.

Want to maximize the placebo effect? A good way to do this, according to a new study, is to tell someone they have a decent chance of getting the real treatment instead of a fake pill, but keep them guessing.

In the study, Parkinson's disease patients given a placebo after being told they had a 75 percent chance of receiving an active drug produced significant amounts of dopamine, a chemical key to the brain's reward system that is scarce in the brains of patients with this disease.

But no dopamine response occurred in patients given placebo after being told they had a 25 percent, 50 percent, or 100 percent chance of getting real treatment.

The findings show that expectations directly regulate the power of the placebo effect by kicking the brain's reward system into gear, probably not just in Parkinson's patients but in a number of different illnesses, such as chronic pain and depression, according to Dr. A. Jon Stoessl of the Pacific Parkinson's Research Center in Vancouver, British Columbia, and his colleagues.

"The greatest form of reward is really to get better, so expectation of improvement is akin to expectation of reward," Stoessl explained in an interview.

Stoessl and his colleagues first demonstrated a relationship between the placebo effect and dopamine release in Parkinson's patients nine years ago. Given dopamine's role in the reward system, he explained, "perhaps it would be important for the placebo effect in other conditions."

In the current study, the researchers used PET scans to examine whether patients' expectations of getting an active drug would be related to the amount of dopamine released in their brain after they took a placebo.

They randomly assigned 35 patients to be informed that they had a 25 percent chance, 50 percent chance, 75 percent chance, or 100 percent chance of receiving an active drug. But all were given inactive placebo.

"There was a substantial amount of dopamine released, but only when the stated probability was 75 percent," Stoessl explained. "What that means is when you're told that the outcome is certain, that there's a 100 percent chance, you don't activate reward pathways. At lower probabilities, you just don't think there's much chance, so you don't activate the reward system either."

The release of dopamine in response to placebo also was closely correlated to the amount of dopamine released in patients' brains when they were given active medication, meaning that the placebo effect would likely add on to the real drug effect.

Parkinson's patients' reward pathways are abnormal given their loss of dopamine, Stoessl said, so results might be different in patients with normally functioning reward systems; for example, they might release dopamine when told they had a 50 percent likelihood of being given an active treatment.

Researchers could use the new findings in designing future studies, in order to minimize the placebo effect and better gauge the effect of a real drug, according to Stoessl.

Stoessl and his colleagues used something called "authorized deception" in the study, meaning that they told participants beforehand that the experiment would involve deception, without going into specifics. Directly after the test, they gave study participants the details on how they were deceived.

While deception has long been used in neuroscience and psychology research -- it can be tough to do research on the placebo effect without misleading patients -- most of the time, study participants are left in the dark, notes Dr. Franklin G. Miller, a bioethicist at the National Institutes of Health in Bethesda, Maryland. This flies in the face of informed consent, the principle that people should be told exactly what a study will entail and why it's being done so they can choose whether or not to participate in an informed way.

According to Miller, who helped develop the concept of authorized deception, Stoessl and his team were on solid ethical ground. "This approach, by alerting subjects to the use of deception, gives everyone a fair opportunity to decide whether or not they want to participate in research that has deception as part of its design," he said.

And the findings also show, he added, that health care providers can take advantage of the placebo effect while treating their patients, without having to lie to them.

"Normally your response to a drug is a product of the drug itself and the context in which it is given," Miller said, adding that this context is, basically, what the placebo effect is all about. Health care providers can harness the placebo effect for their patients' benefit, he said, by "promoting positive expectations and good doctor-patient relationships."

Med Changes Are Not Fun

The first few days after I dropped Effexor and began Pristiq I had a nice little headache that felt like a jackhammer had been set loose at the base of my skull but this was to be expected as Effexor just does not like to go away quietly. So I put up with the headache for about a week then I though that would be it and the Pristiq would do whatever the Effexor did but it turns out I was wrong. So I went from a headache from hell to what felt like a flu whose symptoms were so bizarre, and still are, I began to worry about other little things such as serotonin syndrome. Heat flashes that would come out of nowhere to the point I would have to change my shirt for it was completely drenched, insomnia which pushes my sanity levels to places I rather not talk about for they would sent my butt back to the psych ward and lock the door and perhaps the oddest symptom of all which is my right arm triceps muscle would decide to do a jig for no apparent reasons. In the last week I have been trying to figure it all out and praying that it is going to be over very soon.

So the problem that I am facing is that are the symptoms caused by the sudden stoppage of Effexor or is this craziness part of the initial symptoms of Pristiq or a combination of the two. Pristiq and Effexor are apparently close in makeup so I thought that the transfer would be as smooth as possible minus a few minor hitches but like I said I was wrong. Mentally it has been a neat trick for the first few days I felt like I was floating on a cloud but boy did that disappear in a heartbeat and the world did a nasty 180 which put me in a funk where the sun never shines. Not a lot of fun.

Now some person who believes that the chemicals that I put into my body in order to make my brain smile are completely to blame and they would be right but of course the other side to that argument is without these evil chemicals chances are I would not be writing this blog for dead men don't talk.

The last week and a half or so I have spent creating two brand new shopping sites that have taken hours upon hours to produce but hopefully all of the hard work will pay off in due time. The first one is A Golf Club Review which surprisingly is a review site about golf clubs. The second site is called Chronograph Watches and you guessed it is about chronograph watches.

Sorry for this complete ramble at a 100 miles per hour my Seroquel is not working too well probably due to me sweating the darn med out at a fantastic rate. take care

Mental health experts ask: Will anyone be normal?

An updated edition of a mental health bible for doctors may include diagnoses for "disorders" such as toddler tantrums and binge eating, experts say, and could mean that soon no-one will be classed as normal.

Leading mental health experts gave a briefing on Tuesday to warn that a new edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM), which is being revised now for publication in 2013, could devalue the seriousness of mental illness and label almost everyone as having some kind of disorder.

Citing examples of new additions like "mild anxiety depression," "psychosis risk syndrome," and "temper dysregulation disorder," they said many people previously seen as perfectly healthy could in future be told they are ill.

"It's leaking into normality. It is shrinking the pool of what is normal to a puddle," said Til Wykes of the Institute of Psychiatry at Kings College London.

The DSM is published by the American Psychiatric Association (APA) and contains descriptions, symptoms, and other criteria for diagnosing mental disorders. It is seen as the global diagnostic bible for the field of mental health medicine.

The criteria are designed to provide clear definitions for professionals who treat patients with mental disorders, and for researchers and pharmaceutical drug companies seeking to develop new ways of treating them.

Wykes and colleagues Felicity Callard, also of Kings' Institute of Psychiatry, and Nick Craddock of Cardiff University's department of psychological medicine and neurology, said many in the psychiatric community are worried that the further the guidelines are expanded, the more likely it will become that nobody will be classed as normal any more.

"Technically, with the classification of so many new disorders, we will all have disorders," they said in a joint statement. "This may lead to the belief that many more of us 'need' drugs to treat our 'conditions' -- (and) many of these drugs will have unpleasant or dangerous side effects."

The scientists said "psychosis risk syndrome" diagnosis was particularly worrying, since it could falsely label young people who may only have a small risk of developing an illness.

"It's a bit like telling 10 people with a common cold that they are "at risk for pneumonia syndrome" when only one is likely to get the disorder," Wykes told the
briefing.

The American Psychiatric Association did not immediately respond to a request for comment.

The scientists gave examples from the previous revision to the DSM, which was called DSM 4 and included broader diagnoses and categories for attention deficit hyperactivity disorder (ADHD), autism and childhood bipolar disorders.

This, they said, had "contributed to three false epidemics" of these conditions, particularly in the United States.

"During the last decade, how many doctors were harangued by worried parents into giving drugs like Ritalin to children who didn't really need it?," their statement asked.

Millions of people across the world, many of them children, take ADHD drugs including Novartis' Ritalin, which is known generically as methylphenidate, and similar drugs such as Shire Plc's Adderall and Vyvanse. In the United States alone, sales of these drugs was about $4.8 billion in 2008.

Wykes and Callard published a comment in The Journal of Mental Health expressing their concern about the upcoming DSM revision and highlighting another 10 or more papers in the same journal from other scientists who were also worried. DSM 5 is due to be published in May 2013.

Depression has long been associated with darkness and grey skies, but a new study suggests there might actually be a scientific basis for these cultural motifs.

A new paper published in Biological Psychiatry this month suggests people who are clinically depressed have difficulty detecting the contrast between black and white, which means that while they’re feeling blue the world might actually look dull or grey.

By attaching electrodes to the lower eyelids and ears of 40 depressed patients and 40 control subjects, lead author Dr. Ludger Tebartz van Elst and his team measured the electro-physiological response of the retina to flickering black-and-white checkerboards.

The participants who were clinically depressed had difficulty perceiving the contrast gain as the squares changed from white to black.

Tebartz van Elst, head of the section for neuropsychiatry and deputy director of the Clinic for Psychiatry and Psychotherapy at Germany’s University of Freiburg, says it is difficult to say what this might look like for individual people, but that his best guess is that it is similar to seeing a picture in which the contrast has been reduced.

“You still see the image, you still see the person, you still recognize Bill or Bob or Jane, or whoever you have pictured, but the precise way of perception changes,” he says.

Tebartz van Elst says the relationship between depression and poor black-and-white contrast perception might exist because receptive fields in the retina that are critical for perceiving contrast involve dopamine, one of the key neurotransmitters involved in depression.

Tebartz van Elst says he and his team have discovered an “an objective marker of the subjective state of depression” which could have implications for diagnosis and treatment — particularly if studies on patients with bipolar disorder or schizophrenia produce different results.

Dr. Mark Berber, a psychiatrist at Markham Stouffville Hospital, professor at Queen’s University and lecturer at the University of Toronto, says his patients who struggle with depression often say things seem bleak; when they’re well, they say the world has a more vibrant colour.

“They notice the sky is blue, the grass is green,” he says. “When you’re depressed, everything seems grey and black.”

This phenomenon has long been noted in literature.

University of Toronto English professor Carroll Balot says that before the rise of modern science, melancholy was attributed to “black bile,” one of the four humours, believed to cast a shadow over the mind.

She is studying Geoffrey Chaucer’s The Book of the Duchess, written in the early 14th century in middle English, in which Chaucer speaks of a melancholy state in which nothing is “desirable or hateful; all is alike good to me.”

Balot says this is an early example of a melancholy artist as lacking the “. . . ability to differentiate between good or bad, or beautiful or ugly, or black or white.”

Tebartz van Elst and his colleagues believe that when depression fades, the patient’s ability to detect contrast should return.

This experience was noted by Winston Churchill, who called his depression “the black dog.” In 1911, he wrote to his wife that his black dog had left him, at least for the time being.

“All the colours come back into the picture,” he wrote.

As mentioned in a previous post the generic effexor that I was taken was suddenly being managed by a different company and it turns out the med is not quite the same as my depression began to take off again. Long story short I saw my mental health doctor and the decision was made to go from 150mg Effexor to 50mg of Pristiq. I have never been on this particular med and I was warned about brain zaps for the first couple of days due to Effexor withdrawal but since Pristiq is close to the makeup of Effexor hopefully the withdrawal will soon past. So now my brain gets to adjust to another new med which is never fun. My Seroquel dose was played with as my sleeping ability has been interrupted so by changing the times it may help
