PIP implant removal

The PIP breast implant manufactured by the now defunct French company, Poly Implant Prosthese, has been surrounded by scandal and controversy over the past two years. It was discovered that the company had been fraudulent about their manufacturing process, while manufacturing these implants with non-medical grade silicone. This was at one time the third leading manufacturer of silicone breast implants and some 300,000 women worldwide are affected by this information. Fortunately, the implants were never FDA approved in the U.S. and women in the U.S. would have had to have the implants placed elsewhere or used without approval.

These implants are found to have a very high rupture rate as much as 5 – 10 x that of silicone implants used in the U.S. The type of silicone used in these implants has not been studied to determine its safety if there is a rupture. For this reason, there has been a general consensus that women with these implants should have them removed with replacement of approved implants as desired.

The lengths that people will travel for plastic surgery

It is amazing how small the world has become. Today I was answering some emails from recent out of town patients of ours and it struck me how many out of town plastic surgery patients we see. Currently over 1/3 of our patients are from out of the state of Utah, some from out of the country. In the past three months we have operated on 3 patients from New York, one from Germany, one from Canada, one from Alaska, one from Washington, one from Kuwait, and dozens from the surrounding states of Idaho, Wyoming, Colorado, California and Nevada.

Most of these patients find us through word-of-mouth (1/3) or internet searches (2/3). Most of our out of town patients travel for those procedures we do best; mommy makeover, breast augmentation, breast lift, breast reduction, tummy tuck, and liposuction. Most stay in town for 2-10 days depending on the procedure.

My staff has a lot of experience with out of town travel for plastic surgery. With a little pre-planning, the steps from consultation to surgery to recovery can be efficient and smooth.

Large breast implants can lead to problems

For patients considering breast augmentation, one of the most common tips they will hear from previous patients is “make sure you don’t go too small”. This is true… if women are unhappy about their chosen size of implant it is most frequent that they wish their breasts were larger.

Let me offer the counter piece of advice, “larger is not always better”. The ideal implant size chosen should match the patients body type and frame. Not only will it look better, it will feel better and the long term results will be more durable.

There are more risks, short and long term, with large breast implants including:

Loss of overlying breast tissue volume. This can lead to more implant visibility, loss of size, and rippling in the long term.
Changes in underlying rib cage shape. This can lead to loss of apparent breast size as the implants hide in the chest a little. It can make secondary surgery more difficult.
Skin stretch and premature droop. This depends a lot on the type of skin and elasticity that a patient has.
Larger breast implants can cause the breasts to have an unusual “augmented” appearance. The breasts can appear more rounded and globular shaped.
There is a higher risk of implant malposition and displacement with larger breast implants.

This is not to say that every woman should choose small breast implants. It is to say that when choosing the breast implant size factors other than how large you would like to be must be considered.

plastic surgery in the movies

Plastic surgery isn’t what is portrayed in the movies and on television. Drugs are not smuggled in breast implants, plastic surgeons are not surgically disguising criminals. The “reality” of TV shows like The Swan and Extreme Makeover is that it is never the goal of plastic surgeons to make a patient virtually unrecognizable to their own friends and family after a series of surgeries.

There is a lot of mystique surrounding plastic surgery and I guess it makes for good film and TV. A google search on the topic interestingly reveals that movies and TV shows about plastic surgery themes is not new dating back to 1928.

The Broken Mask (1928) – silent, black and white. A plastic surgeon corrects a man’s scarred face, then destroys it out of jealousy.

Black Dragons (1942) – Black and white war drama. Japanese plastic surgeon operates on Japanese agents to make them pass as American.

Dark Passage (1947) – Humphrey Bogart plays a convict who is framed for the murder of his wife. He has plastic surgery to find the real killer.

The Girl Most Likely to… (1973) – A girl becomes much prettier after plastic surgery following a car crash. She exacts revenge on previous high-school tormentors.

A Stolen Face (1952) – A plastic surgeon transforms a female convict to look like a beautiful pianist he is in love with.

Rabid (1977) – Horror about a woman who has a new plastic surgery procedure after a motorcycle accident. She develops an unusual deadly lesion in her armpit and taste for human blood.

Doc Hollywood (1991) – Michael J. Fox plays a hot-shot Beverly Hills plastic surgeon.

Breast Men (1997) – Fictional look at two plastic surgeons played by David Schwimmer and Chris Cooper who invent the breast implant in Texas in 1960.

Face Off (1997) – A criminal genius played by Nicholas Cage and FBI agent played by John Travolta trade faces through an experimental plastic surgery procedure.

Extreme Makeover (2002-2007) – Reality TV. Unattractive individuals volunteered to have multiple cosmetic surgery procedures.

Nip/ Tuck (2003-2010) – Far fetched drama about the lives and practices of two ethically challenged plastic surgeons

Dr. 90210 (2004-present) – Reality TV about the lives and practices of a number of plastic surgeons. Interestingly the most recognizable plastic surgeon on the series is not a board certified plastic surgeon. I find that an interesting choice for E!.

The Swan (2004) – Reality TV. Unattractive women underwent radical cosmetic surgery transformations.

Bridalplasty (2010) – Reality TV. Twelve women compete for a dream wedding and plastic surgery procedures.

Just Go With It (2011) – Adam Sandler plays a Beverly Hills plastic surgeon opposite Jennifer Aniston.
