<description><![CDATA[Assumes on-going primary responsibility for an assigned caseload of patients in order to coordinate, implement and evaluate patient plan of care in the patient's home. Coordinates activities with other health team members to ensure optimal patient care and communication of pertinent findings. Reviews patient insurance information, identifies appropriate billing source, and bills appropriate insurance based on patient needs.
