<p>We must understand that with a new rheumatoid arthritis  diagnosis, the patient with be given and taught a lot of information  with regards to the management of joints, pain, energy conservation,  joint protection, equipment and tools etc &#8211; there is a limit of how much  a patient can understand and retain, so it&#8217;d be a good idea for  continuity of care and protection information by creating an information  sheet or booklet. This booklet should be informative in terms of  definition, exercises, what activities to avoid, what equipments to  purchase and how to use them.<span id="more-306"></span></p>
<p>Occupational therapists and  physiotherapists designing their own booklets should consider the  following segments to be included in their rheumatoid arthritis  management booklet:</p>
<p>1. <strong>Biomechanics and Body Positioning</strong></p>
<p>Patients  are educated and taught to keep a logbook, a diary of sorts, to jot  down and note what positions are helpful and makes them feel energized  and which positions do not, their possible causes and their possible  solutions</p>
<p>2. <strong>Breaks and Resting</strong></p>
<p>Patients are informed  to measure the length, frequency and duration of any activity or tasks  that they choose to engage in, understand and jot down the rest periods,  document any pain or discomfort and fatigue they experience after the  activity. This should be as detailed with data as possible, so that we  can plot and schedule activities accurately</p>
<p>3. <strong>Analysis Of The Task Or Activity</strong></p>
<p>The  occupational therapist then teaches the patients how to analyze the  activities they have chosen to participate in, breaking down the tasks  into components of mental, physical aspects, determining when and where  to take their rests; body mechanisms and postures, environmental aspects  such as temperature, work bench heights, how far or near the equipments  or tools are. This details will be keyed in daily into a weekly time  table, and tasks are spread out according to their difficulties.</p>
<p>4.<strong>Protecting Their Joints</strong></p>
<p>Patients  to understand the difference between common joint pains from those  which are caused by arthritic joint pains post overactivity. For common  joint pains, the pains usually wear off after about 45 minutes with  rest. This segment should involve the principles of joint protection as  well.</p>
<p>Since 2008, we have been providing physiotherapy and occupational therapy services to solve rehabilitation issues such as hands and upper limb injury  rehabilitation; prevent, manage and rehabilitate falls in the elderly;  rehabilitate patients with hip and knee fractures and replacements; as  well as give talks and workshops to prevent back, neck and hand injuries  at work since 2008. Often they end up trying too  hard and injures or aggravates their affected joints.</p>
<p>Energy  conservation, at all stages of rheumatoid arthritis is important, and  when combined with the joint protection program, will result in better  preserved and protected joints for the patient with rheumatoid  arthritis.<span id="more-305"></span></p>
<p><strong>Energy Conservation Program</strong></p>
<p>1. in between  tasks and activities for the waking day, at least 60 minutes should be  purely for resting. This is best when scheduled right in the middle eg  work 9 am to 1 pm, rest 1 &#8211; 2 pm, continue work from 2 pm &#8211; 6 pm.</p>
<p>2.  the needs to be a balance struck between activity and rest, we can call  this microbreaks eg working for 20 minutes continuous before taking a  short stretch for 1-2 minutes to rest the joints and muscles</p>
<p>3.  scheduling and pacing work, that there is an adequate mix of heavy and  light work at a time, not a pure stretch of 2 hours of heavy work etc</p>
<p>4.  whenever possible, use leverage to help. This can be in the form of  technology such as using the computer to type instead of using pen and  pencils, using a car to travel instead of walking long distances, or  even asking for manual help when required to carry heavy objects</p>
<p>5.  plan and arrange the physical environment eg at home or at the work  place for equipment, materials, items to be within easy reach, waist  height, not overly high or low to minimize climbing and crouching</p>
<p>6.  it would be best if the patient with rheumatoid arthritis be allowed to  continue her past occupations in a seated manner rather in standing  manner, to preserve and protect her joints and energy as much as  possible</p>
<p>7. patient should educated and understand that they  should accept help whenever possible or necessary especially when it is  heavy and difficult to maneuver</p>
<p>8. and to maintain focus primarily  on the most important tasks of their jobs or roles, and to remove and  eliminate tasks or activities that are not pertinent for them to do eg  being a mother doesn&#8217;t mean she has to help her children (assuming  they&#8217;re young) with their school work or carry their heavy bags or books</p>
<p>Since 2008, we have been providing physiotherapy and occupational therapy services to solve rehabilitation issues such as hands and upper limb injury  rehabilitation; prevent, manage and rehabilitate falls in the elderly;  rehabilitate patients with hip and knee fractures and replacements; as  well as give talks and workshops to prevent back, neck and hand injuries  at work since 2008. For this reason, the joint  protection program was conceptualised and further developed for our  patients. The main goals of this program is to first and foremost  prevent aggravation and secondly, to relieve pain and minimise deformity  from occurring.</p>
<p><strong>Principles for the joint protection are:</strong></p>
<p>1.  using adequate and correct body biomechanics to perform tasks eg using  larger or stronger joints to perform activities, using the hips and  knees for lifting objects rather than using the back, pushing objects  instead of pulling, using a trolley instead of carrying heavy items etc<span id="more-304"></span></p>
<p>2.  each task or activity is to be done in a stable position, eg when one  is stable and standing balanced and squarely BEFORE lifting or pushing  object, moving closer to an object before lifting it</p>
<p>3. spread the  load over an area (P=F/A), the wider the area spread, the less the  pressure, so using our forearms to support lifting a heavy object would  be easier than to just carry it with pure fingers</p>
<p>4. use the full  range of motion for joints when performing a task eg when cooking or  cutting, to use the full range of the shoulders and elbows instead of  just using elbow flexion-extension in a locked position to cut, that  will induce more pressure on the elbow joint</p>
<p>5. to not participate  in activities that encourage or aggravate the ulnar drift in the wrist  eg cutting repetitively, using the scissors excessively, using small  handles, wringing cloth etc</p>

Comprehensive occupational therapy assessment would include physical,  physiological, psychological, social and environmental perspectives of  the patient with rheumatoid arthritis. It is often at this stage also  that many of the patients would probably have stopped any form of paid  employment, except for housewives who would still be doing household  chores.</p>
<p>The things that the hand occupational therapist will be  looking out for includes power, presence of deformity, any signs of  subluxation, skin temperature, pain and range of motions will be  assessed. Given the fact that rheumatoid arthritis has its cycles of  exarcebations and remissions, it&#8217;d be good to keep an assessment mindset  over at least 3 sessions to ascertain the hand and its function. It&#8217;d  be good also to breakdown the assessment over 2-4 sessions depending on  patient&#8217;s ability to tolerate each session, as not all patients would  like to keep travelling to the clinic.<span id="more-303"></span></p>
<p>The hand occupational  therapist would then perform a series of checks, one of them is the  informal interview. The results of the informal interview may be rather  inaccurate as the patient might refer to a time prior to the onset of  the disease, and since the disease is progressive they might not be very  accurate as well. It&#8217;d be best to combine a variety of assessment tools  eq quantitative and qualitative approach to have a more holistic idea  to their hand function.</p>
<p>Essentially, it is best for the hand  occupational therapist to perform a practical assessment task on the  patient, covering the activities of daily living (ADL).</p>
<p><strong>The ADL activity assessment should include but is not limited to:</strong></p>
<p>1. dressing: able to wear and take off own clothing, including undergarments and prostheses, if any<br />
2. personal hygiene: brushing teeth, wiping/washing face, handwashing<br />
3. toileting: ability to perform voiding activities<br />
4. feeding: feeding and drinking during meal times<br />
5. mobility: walking, stairs management, transfers<br />
6. housework: cleaning, laundry, marketing, money management<br />
7. communications: using a phone to SMS or make calls, writing, typing etc.</p>

<p><strong>Active and Passive Treatment in Physical Therapy</strong></p>
<p>Active  treatment involves certain therapeutic exercises. These help to stretch  and strengthen the back muscles. The therapists show the proper way of  doing these exercises and constantly monitor the improvements.<span id="more-302"></span></p>
<p>Passive  treatment is more advanced as it is based on scientific study. Physical  therapy treatment now includes pain relieving modalities such as TENS  (Transcutaneous electrical nerve stimulation) and ultrasound. Patients  would be treated with heat and cold therapy along with massages and  joint mobilization. Lumbar stabilization is a program that is meant for  those who complain of lower back pain.</p>
<p>In almost all the cases, physicians customize the treatment program since the individual requirements of patients differ.</p>

<p>Patients newly diagnosed with rheumatoid arthritis needs to be  treated with respect, and we as health care professionals need to  recognize and be able to identify and recognize that they (the patients)  have emotions, ideas, values, feelings, hopes &#8211; by doing this we affirm  him or her, their identities, their beings, creating a positive effect  on their self image and self esteem.</p>
<p>For this to happen smoothly,  we may have to setup the environment in such a way that it facilitates  such an encounter, or we facilitate the patient using guided questions,  or getting them to participate in &#8220;open&#8221; activities that will help them  to express. Many times, a simple listening ear is more than enough. From  here, we can identify and check if they have any wrong understandings,  or had apostrophized their rheumatoid arthritis diagnosis, and correct  them.<span id="more-301"></span></p>
<p>This does not mean that we belittle, ignore or minimize what  they feel, think or believe, in fact even if the diagnosis and  prognosis is not as good as we would like it to be, many patients with  rheumatoid arthritis do not progress into severe disability and/or  impairment. With compliance to joint protection program, energy  conservation program as well as consistent exercise, many end up with  fairly good control and results post diagnosis.</p>
<p>We must not  overlook the patient&#8217;s spouse, family and relatives &#8211; they may need help  to, to understand the disease, it&#8217;s potential progress, their  prognosis, living and adapting to the disease. Of course, they may have  different attitudes and personalities to managing loss and pain, some  may be protective, some may be resentful, some angry, some appears to  have given up hope etc. Some may give up, as the fear of long term care  for a person with rheumatoid arthritis may seem daunting, and future  plans totally altered.</p>
<p>Some partners, spouses and families  responds with utmost love, appreciation, dedication and being positive,  some are unable to cope, some withdraw, some even may choose to separate  or divorce. There are many instances of role reversal, most often when  the previous sole worker is diagnosed with rheumatoid arthritis &#8211; this  sometimes becomes difficult for both partners or spouses to accept,  causing loss of perceived roles and self esteem, even resentment and  anger for each other.</p>
<p>This plus financial costs of injuries as  well as loss of potential incomes in the future, and this becomes a  little more complex and painful. Social workers will be invaluable here,  for family counseling, and to point in the direction of funding  availability and help. Caregivers must be adequately prepared and  trained, and educated on the possibility of social isolation, burnout  and stress. There needs to be a scheduled time for rest, relaxation and  recreation.</p>
<p>Promotion of independence and mobility will be continued with by the occupational therapist.</p>

By the time the person  suffering from the disease&#8217;s progression to reach a stage where  operation is required, they would have gotten over the initial shock,  but will still be vulnerable to bouts of moodiness, depression and  helplessness.</p>
<p>These are common and usual response of people  towards having to live with pain and deforming disease. Sometimes,  antidepressants are required to manage sleep, loss of weight etc.<span id="more-300"></span></p>
<p>The  person with the rheumatoid arthritis disease may feel overwhelmed with  the disease, but may internalise it and keep it inside themselves. These  people need to be encouraged to express their feels, fears, hopes, in  order to accurately help them define and manage their fears, regardless  if they were perceived or real. An example is if a person feels that  they have lost the ability to handle or the physical ability to maintain  the joints integrity, they might feel frustrated over the apparent loss  of control, but they perceive that they aren&#8217;t supposed or allowed to  be angry as the people around them are already doing their best to make  him or her as comfortable as they can.</p>
<p>They may develop body image  disorders, or have body image issues as they would have to perceive or  wait in fear &#8220;when or in case&#8221; their joints change in shape,  functionally or esthetically. What will couple this is if pain comes  into the picture, of if they are placed on steroids, which often cause  fluid retention and the effect of weight gain and puffy faces.</p>
<p>The  occupational therapist may then create a &#8220;safe room&#8221; or where the  patient with rheumatoid arthritis will be allowed to express their  feelings through verbal or written forms of communication, directed or  undirected by the trained occupational therapist. They need to be  assured and re-assured that the feelings that they have are normal, a  part of the the grieving and loss process.</p>
<p>Patients needs to  understand that, and to be respected and given the space, chance and  ability to grieve over his or her loss (real or perceived) of function,  mobility and independence. By this, we can help them to overcome their  mobility and functional problems.</p>

<p>Teeth grinding causes may be  physical or psychological. Doctors suspect that stress is particularly  to blame because it disrupts a person&#8217;s sleep. People may subconsciously  grind their teeth in an effort to vent anger, anxiety and stress.  Dislocated jaw joints or misaligned teeth are physical issues that can  cause excessive grinding. Vitamin deficiencies can also lead to bruxism.  Low levels of calcium in the body can cause involuntary movements and  muscle contractions. A deficiency of magnesium can also cause muscle  spasms leading to clenching and grinding.<span id="more-299"></span></p>
<p>There are a number of  things people can do to stop teeth grinding. Techniques to relieve  stress can be extremely helpful. These include relaxation methods,  massages and meditation. To prevent damage from teeth grinding in sleep,  mouth guards are the ideal product. There are also mouth guards  available for use during the day, and they can be custom-made by a  dentist. A dentist can correct any dental issues that may be causing the  grinding. When grinding is caused by a nutrient deficiency, a  multivitamin or single vitamin can be used to boost the level of  nutrients in the body. For more severe cases where there is damage to  the joints or jaw bones, surgery can be used to perform repairs on the  joints and bones.</p>
