Insel" href="http://en.wikipedia.org/wiki/Thomas_R._Insel" rel="wikipedia">Thomas Insel</a>, Director of the National Institutes of <a class="zem_slink" title="Mental health" href="http://en.wikipedia.org/wiki/Mental_health" rel="wikipedia">Mental Health</a>, joins the growing group of leading psychiatrists who address the neuroscientific inadequacy of the current diagnostic system. <blockquote><p>In a potentially seismic move, the <strong><a class="zem_slink" title="National Institute of Mental Health" href="http://www.nimh.nih.gov/index.shtml" rel="homepage">National Institute of Mental Health</a></strong> – the <em>world’s biggest mental health research funder,</em> has announced only two weeks before the launch of the <a class="zem_slink" title="DSM-5" href="http://en.wikipedia.org/wiki/DSM-5" rel="wikipedia">DSM-5</a> diagnostic manual that it will be “re-orienting its research away from DSM categories”.</p>
<p>In the announcement, NIMH Director Thomas Insel says the <em>DSM lacks validity</em> and that “patients with mental disorders deserve better”.</p>
<p>This is something that will make very uncomfortable reading for the <a class="zem_slink" title="American Psychiatric Association" href="http://www.psychiatry.org" rel="homepage">American Psychiatric Association</a> as they trumpet what they claim is the ‘future of psychiatric diagnosis’ only two weeks before it hits the shelves.</p></blockquote>
<p><span style="font-size: medium; font-family: trebuchet ms,geneva; color: #333399;">Cognitive Inquiries Matter</span></p>
<p>And the report at Mind Hacks further breaks down the cognitive details, sadly lacking in DSM 4/5 ADHD diagnosis. If ADHD is, as we know, a <strong><em>cognitive</em></strong> process, why do the &#8220;diagnosticians&#8221; continue to overlook cognitive inquiry?</p>
<blockquote><p>As an alternative approach, Insel suggests the Research Domain Criteria (RDoC) project, which aims to uncover what it sees as the ‘component parts’ of psychological dysregulation <strong><em>by understanding difficulties in terms of cognitive, neural and genetic differences.</em> </strong>[Sound familiar?]</blockquote>

It&#8217;s obvious: if you don&#8217;t get that Ice Fishing house off the lake within an appropriate time frame, your friendly cabin will become an <strong>Unlucky 13</strong> floating quietly in the Spring thaw, or touring deep to fully experience the bottom of the lake. So it is with ADHD meds: get the timing right and you&#8217;re golden, miss the<em> built-in timing</em> and you will be lucky to stay afloat.</p>

<p>I&#8217;ve been teaching my colleagues about the Reality details in this <em><a title="ADHD Insights: Reality Changes"><strong>ADHD, OCD and Anxiety</strong></a></em> video since 1996 when I first recognized the <a title="ADHD Insights: Social Anxiety Revisited" href="http://www.corepsych.com/2012/10/adhd-insights-social-anxiety-revisited/">prefrontal cortical</a> imbalances associated with <strong><a class="zem_slink" title="Executive dysfunction" href="http://en.wikipedia.org/wiki/Executive_dysfunction" rel="wikipedia">Executive Dysfunction</a>: <a title="ADHD Insights: ADHD, ADD, AAD, EFD" href="http://www.corepsych.com/2012/09/adhd-insights-adhd-add-aad-efd/">Cognitive Anxiety.</a> </strong>  No, you won&#8217;t find <strong>Cognitive Anxiety</strong> in the <a class="zem_slink" title="Diagnostic and Statistical Manual of Mental Disorders" href="http://en.wikipedia.org/wiki/Diagnostic_and_Statistical_Manual_of_Mental_Disorders" rel="wikipedia">DSM 4</a> <strong>or 5</strong>, those antique maps are outdated by about 20 years of brain studies. No, in 1996 we didn&#8217;t call these mind-details Executive Dysfunction, but we did call this one Cognitive, Thinking Anxiety &#8211; even back then.</p>

Yesterday this video <em>passed 19,000 hits on YouTube</em> &#8211; it looks like many more are paying attention, actually thinking about thinking.</p>

But we lost the automatic article updates by email, so are moving to <a class="zem_slink" title="RSS" href="http://en.wikipedia.org/wiki/RSS" rel="wikipedia">RSS</a> in your own specific Reader. <p>Let&#8217;s all make a <strong>New Year&#8217;s Resolution</strong> to honor the reality of neuroscience &#8211; of those new brain maps &#8211; for all of psychiatric treatment. Without maps too many folks careen off the tracks that should set up a safe arrival. Let&#8217;s make 2013 a different year by asking better questions &#8211; improving both working knowledge and useful information.</p>
<p>But is &#8220;all of psychiatric treatment&#8221; too sweeping in complexity? &#8211; Then reduce your target: just consider ADHD, the most misdiagnosed, mismanaged, maligned, source of psychiatric &#8211; mind gossip out there.</p>
<p><span style="font-size: medium; font-family: trebuchet ms,geneva; color: #333399;">Just What Did You Expect?</span></p>
<p>About 90% of stimulant medication problems are based on one simple challenge: <a href="http://www.corepsych.com/2012/10/adhd-insights-lets-change-the-rules/">Limited to no expectations</a>. Imprecision to often becomes the current standard of care. From <strong><em>Great Expectations</em></strong> to <em><strong>No Expectations</strong></em> the meds are delivered far too often without even remotely considering specific objective targets. &#8220;Just tell me how you feel&#8230;&#8221; addresses <em>affect, feelings</em>, not <em>cognitive challenges</em>. Clear <strong><em>Rules</em> </strong>and much improved expectations provide more deliverable, improved ADHD treatment outcomes.</p>

<li>Just <strong>exactly when to <em>medications start to work</em></strong>? Meds might not even touch the bottom of <a href="http://www.corepsych.com/2012/09/adhd-medications-supplements-therapeutic-window/"><strong>The Therapeutic Window</strong></a>.</li>
<li>Remember: If stimulant meds are added, but then <a href="http://www.corepsych.com/2011/06/psych-meds-time-titration-tachyphylaxis/"><strong><em>don&#8217;t start working until 2-3 hours later</em></strong></a>, then the meds are likely too high in dosage.</li>
<li>If the crash in the PM matches the expected <a href="http://www.corepsych.com/2012/10/adhd-insights-short-acting-stimulants/"><strong>DOE | Duration of Effectiveness </strong></a>but the crash is emotional at 3-4 PM the look for depression as a comorbid challenge.</li>
