It's also possible to slowly lose more weight than is advisable to be healthy. A colleague of mine did this recently. She lost around 40 pounds over the course of a year. We lived in fairly close quarters, so I saw some of her eating habits. She ate healthy and worked a couple days a week. At 20lbs down she looked great! Then she kept going and started to look less than great. I think that her weight just wasn't right for her body type--she is a pear, but she was trying to get the body of a ruler, if you know what I mean. I don't think she had an ED or worked out obsessively. I think she just gradually restricted her calorie intake.
