Sizing It Up : Breast implants: Should you get them or not?

During her teens, Anna, 29, of Placentia, filled out a 32D-sized bra. As she got older, however, and after some weight fluctuations, she was dissatisfied to find her breasts had decreased by at least one cup size. Missing her old curves, Anna decided to take matters into her own hands - or rather, the hands of a plastic surgeon.

"I was originally going to go in for a lift, but the advising doctor told me that a better solution would be to replace the space where I had lost mass," Anna says. "I think the doctor did a fantastic job, [but] I also think my implants were too big. I feel embarrassed about how large my breasts are: 32DD."

Why do women opt for them?
Naturally, there are both advantages and disadvantages to having a breast augmentation. For women who have always been embarrassed about their lack of cleavage, or whose breasts, like Anna's, have changed in shape and size, surgery may be an attractive option. Many women report that having the procedure has helped raise their self-esteem.

A lot of women seeking breast surgery are looking to make their bodies more proportionate, according to Dr. Michael Niccole, a board-certified plastic surgeon at CosmetiCare in Newport Beach who has 27 years of experience performing breast surgery. "These women may have very wide hips, but small upper torsos," Dr. Niccole says, explaining that breast implants can create a more balanced look.

Another large portion of breast surgery patients have had children and want to restore the appearance of their breasts before pregnancies and breastfeeding. "The breasts can deflate after having kids - women may have lost 25-30% of breast volume," he says. "A third category of women are as flat as a board and want to look more feminine."

What could go wrong?
"Some women have an infection or bleeding," says Dr. Hootan Daneshmand, who's been practicing cosmetic surgery for 11 years and performs about 300 breast augmentations a year at the Silhouette Plastic Surgery Institute, with offices in Newport Beach and Foothill Ranch. "The risk of bleeding is about 2-3%, while the infection rate is 1-2%. About 5-7% of patients will experience implant firmness in the first year after the surgery because of capsule contracture."

You should also note, Dr. Daneshmand says, that breast augmentations don't necessarily last forever. If you get implants in your 20s, you will likely need to have one or a couple of additional procedures done later in life, often to correct implant leakage. "A small hole or tear in the implant can cause it to deflate, and you might notice that one side looks smaller than the other."

Find a qualified surgeon
Dr. Niccole estimates that 98-99% of women who get breast implants are happy with the results - providing that they choose an experienced, certified plastic surgeon.

This is something Stephanie, 27, has been taking into account. The Fullerton resident wants to get a breast augmentation to "lift and increase" her breasts. "The only thing I've done is talk to my friends about successful doctors," Stephanie says. "There are so many 'chop shops' these days that you have to be careful."

They're not cheap, either!
There's no getting around the fact that breast augmentations are expensive, costing anywhere from $3,000 to $18,000. While it's great to get a good deal on, say, milk at the supermarket, don't go looking for "Buy one boob, get one free" deals - this is hardly a time to pinch pennies. "If it's too cheap, you will most likely be sacrificing quality," Dr. Daneshmand warns.

Silicone or saline?
For the past 14 years, silicone breast implants have been banned, as the FDA requested research into possible risks and side effects. Now, a new, safer version of the implant has been introduced, presenting women with a choice: silicone or saline? "Silicone gel feels more like a natural breast and there's no rippling or wrinkling, as there can be with saline," says Dr. Kelly Gallego of Gallego Plastic Surgery in Irvine.

One downside with saline is that it's often harder to know if the implants have ruptured because saltwater will be absorbed by the body. Silicone implants, however, are harder to replace and have been found to have a higher rate of capsule contracture, which occurs when the scar around the implant begins to squeeze and harden the breast. "And silicone gel implants are more expensive - about $1,000 more," Dr. Gallego says. "Also, you can't place the incision in the armpit or belly button. Instead, you often (have) it around the areola - the dark area surrounding the nipple."
But judging by statistics, the procedure shows no signs of decreasing in popularity anytime soon. "Women," Dr. Niccole concludes, "want to have perfect breasts."

10 Ways to Prevent Breast Cancer

October is Breast Cancer Awareness Month, but we should not wait until this month to implement prevention methods. With that said I wanted to share some tips that can help reduce your chance of developing breast cancer.

There is a 1 in 8 chance of developing breast cancer in your life, but hopefully with this plan the odds will change. Although there are many drugs on the market and much research is being done, the odds of a woman getting cancer in her lifetime aren't shrinking.

It seems that diet has a lot to do with the handling of this disease and it is something we can definitely control. Diet has be proven to be the most effective way to reduce your risk. There are many other suggestions such as exercise but today we will focus on 10 simple ways you can change your diet and start protecting yourself today.

1. Reduce your Fat - Limit your fat intake by 20 percent of your overall caloric intake. Research has proven that a diet high in fat will increase the risk of breast cancer. High fat diets have also been proven to produce chemicals in the intestine that bacteria convert to carcinogenic estrogens. These estrogens can then be stored in the fatty tissue of the breast, making the cells in the area more susceptible to cancer growth.

2. Choose Crunchy Veggies - Eat plenty of cruciferous vegetables such as broccoli, brussel sprouts, cabbage, turnips and cauliflower. It is best to steam them or eat them raw to best preserve their cancer fighting nutrients. These types of vegetables contain sulfurous compounds called indoles, which have shown to help eliminate estrogen from the body and prevent it from triggering the growth of breast cancer.

3. Increase your Fiber - Get plenty of fiber from foods such as vegetables and fruits, legumes and whole grains. The fiber tends to interrupt the metabolism of estrogen and decreases the blood levels of estrogen. The high levels of blood estrogen in the body are close in relation to higher risk of breast cancer. High fiber diets have been shown to reduce the risk of of breast cancer by up to 54 percent.

4. Try Seaweed- Try eating seaweeds such as kelp and nori or consider taking spirulina (blue algae) and chlorella in a glass of juice daily. High consumptions of kelp may explain the lower incidence of breast cancer in Japanese women. Kelp and chlorella have shown to have anti-carcinogenic effects.

5. Soy Secret - It is smart to eat soy products such as tofu, miso or tempeh. Soybeans and other soy products contain a plant estrogen called genistein. This plant estrogen binds to receptors in the breast, making it impossible for potentially cancer causing forms of estrogen to connect with breast cells.

6. Shop organic - Buy organic foods such as fruits, vegetables, meates, dairy and grains when ever possible. Organic products do not contain pesticides that have been linked to breast cancer. Dairy products and meats that are certified organic do not contain any growth hormones, such as bovine, a chemical usually fed to cows. This chemical has been shown to promote the growth of breast cancer cells.

7. Eat Fish - Eat at least 3 servings of cold water fish a week. Some cold water fish are tuna, salmon, halibute, cod and sardines. If you can't eat fish you can take fish oil capsules or vegetarian supplements. The omega-3 oils inhibit the effects of compounds known as prostaglandins. Prostaglandins have been associated with inflammation that will suppress the immune systems's ability to identify tumors. A higher consumption of fish and fish oil has been found to reduce the cases of cancer.

8. Choose the right oils - Avoid canola oil, safflower oil, sesame oil and margarine. It is best to use virgin and extra-virgin olive oil when cooking. Flaxseed oil is also very wise to use as long as it is not heated; can be added to foods already cooked. Olive oil, a monounsaturated fat has been linked to lower rates of cancer. Trans fats also called hydrogenated oils mainly found in margarine have been linked with high rates of cancer.

9. Add phytonutrients to your diet - Introduce nuts, legumes, seeds as well as fruits and a variety of vegetables to your diet. These particular foods contain phytonutrients. Phytonutrients are plant compounds that protect against cellular damage and prevent cancer growth. Cooking with herbs such as dill, which contains a phytochemical called limonene, is important for breast protection and rosemary has antioxidant and anti-tumor properties.

10. Mushrooms - Mushrooms have medicinal properties. Some medicinal mushrooms include the Japanese variety of shiitake and maitake. Studies have shown that maitake mushrooms actually stimulate the immune function and prevent tumor growth. The ingredients in maitake mushrooms do not kill the cancer cells directly, it activates the immune system protecting against breast cancer. Shitake mushrooms have also been found to boost the immune system.

If women with small breasts are found so attractive, sexy, and desirable, why then do large breasts receive so much attention? Women's breasts are almost always viewed in a sexual context, and women with large breasts are perceived as being innately more sexual than other women. Women with large breasts are presumed to desire sex more often and have less control over those desires than other women. I believe, women with large breasts are seen as a socially acceptable outlet for men's sexual desires. As a result of this social expectation, men may desire sex with women with large breasts because they are expected to be readily available to fulfill their sexual needs, even if they aren't interested in pursuing a long term relationship with them. Men don't necessarily want to date and marry women they or society views as sexual.

On the other hand, I believe attractive women with small and average sized breasts are viewed as potential wives and mothers, because they are perceived as less sexual, and by default, more maternal. Society simply doesn't accept the idea of a sexual mother. From a social perspective, it is better not to be known for your breasts, as you are more likely to be considered a desirable partner and someone they can bring home to mom. If this logic holds true, there is a benefit to having small breasts, even if women do not believe it. This obviously creates a conflict for the woman with small breasts who wants to be perceived as sexual, and the woman with large breasts doesn't want to be.

Kym and Jamie are still recovering from the tragic loss of their son, Archie, who died in February after being born 18 weeks premature, and friends have claimed Kym - who has two children from a previous relationship - decided to go under the knife to improve her confidence.

Kym is not the first soap star to opt for a breast augmentation.

About the time the economy began to completely unravel last fall, I wrote a story about the life of a few local prostitutes. The story was prophetically titled "TheReal Girlfriend Experience." I say prophetic because director Steven Soderbergh(Traffic, Erin Brokovich, the Ocean's 11 series) has a new movie out about the life of a New York prostitute during the financial collapse called "The Girlfriend Experience."

The film stars porn star Sasha Grey, whom Rolling Stone calls "the dirtiest girl in the world" in its profile of her in the current issue. Grey is also the reigning AVN Female Performer of the Year (at 21, she's the youngest woman to win the honor). She was in town this week to pimp the movie during Exxxotica Miami Beach. I watched the film (see the trailer here) and caught up with her on Friday at theCatalina Hotel.

I asked her about the research she did for the Soderbergh movie, the Rolling Stone story, her relationship, and how she feels porn fits into the women's lib movement.

We meet in the lobby of the hotel. She is with her 33-year-old boyfriend, Ian, asking the front desk if she is already on the mezzanine level. I am trying not to sweat valuable parts of my body off. The publicist introduces me as "the writer who did that GFE story."

Sasha Grey is part porn star, part businesswoman, part philosophy student. She uses expressions like "clinically speaking" and drops words like "enigmatic" as casually as words like "fucking." She is at once charismatic and distant, pleasant but still cold.

Grey wants people to be more open and comfortable with their own sexual preferences. "All sorts of people like all different things, and we just need to accept that as a society," she tells me. She prefers being choked, slapped (but not on the breasts), and sometimes getting doubly penetrated. And she says so. Sometimes on Tyra, sometimes onOprah, sometimes in Rolling Stone or the Juice, and often in her many, many adult movies. In her first-ever porn scene, she famously told her partner to punch her in the stomach before his climax.

"In our society, we use sex to sell everything. Everything! Right now there's a commercial on TV that's selling kids toys -- some SpongeBob Burger King meal -- is using sex to sell." The commercial in question stars Sir Mix-a-Lot, the architect behind hits like "Baby Got Back." "So we can do that, but we can't openly talk about who we are as sexual beings without feeling ashamed. I think that's ridiculous."

She says one of the reasons she got into this business was to encourage people to feel more comfortable with themselves. "Listen, it's OK to be yourself," she says. "You might have different fantasies or a different sexual interest than your sister or your best friend has, but that's OK. We're all human beings. Everybody's different."

In conversation, Grey references everything from hipster musicians to relatively obscure philosophers to Dr. Alfred Kinsey, the biology professor turned sex researcher from the middle of the 20th Century.

"Dr. Kinsey did a test on people's sexuality, whether they're straight, gay, or somewhere in between. For me, I see the same thing when it comes to sexual tastes and sexual interests."

The second part of her porn name, she says, comes from the term Kinsey used when describing human sexuality: shades of grey. She believes when it comes to sex, there is no right or wrong, good or bad. There is just different. Who could argue with that?

What creates this variety of sexual interests, she isn't sure.

"Is it genetic? I'm not a scientist. I don't want to answer that yes or no. But I do think it's an individual thing. I have best friends who've never had an orgasm. I have best friends that love anal sex, and I have best friends that hate anal sex.

"There's so many different aspects to our sexuality, and we can't talk about them without being vilified. It's 2009; it's time for a change."

I ask her why she thinks the country is so repressive.

"To keep women down!" she jokes. "Nah, I'm being paranoid now." She continues seriously. "It's the last taboo. Maybe it's the little secret everybody keeps tucked away in their pocket. I think it's a very moralistic, Christian way of thinking. I hate to put it that way, but it is. We live in a very Puritanical country. For as far along as we are in technology and the media and as a whole, we're not far along with sex."

She says she feels like women in this country are controlled by shame. At some point, she says, Grey would like to go on a speaking tour, with the message: It's OK to be who you are.

"Shame is bad because it inhibits you," she says. "It's fear. I'm not gonna sit here and say I'm not afraid of certain things in our society sometimes, but we live in such a fear-based society, people are really afraid to be who they are. Whether it's because of what somebody else thinks or the culture around them. Shame stops your personal growth."

She said she hasn't read the Rolling Stone story. "I haven't had time," she says. But when I read her a headline about empowering women "one gangbang at a time," she says, "That's obviously the writer saying that, not me."

She says she wouldn't call herself "a crusader," and she thinks the term "feminist" is useless. "Every woman is a feminist in her own right," she says. "Whether you're anti-porn or pro-porn or somewhere in between, feminism has become such a generalized, watered-down viewpoint. Someone can say, 'I'm a feminist because I believe in sexually empowering women.' That's my view on feminism. Someone else's opinion might be, 'Having sex is just wrong no matter what.' And both sides might call themselves feminists."
